血清人附睾蛋白4检测在肺癌诊断中的意义

摘　　要：目的探讨人附睾蛋白4（human epididymis protein4，HE4）在肺癌诊断中的意义。方法采用酶联免疫吸附试验检测60例肺癌患者和60例肺良性疾病患者血清中HE4的含量，分析其表达差异；采用电化学发光法检测神经元特异性烯醇化酶（neuron—specific enolase，NSE）、细胞角蛋白19片段（cytokeratin 19 fragment，cyfra21-1）、癌胚抗原（carcinoembryonic antigen，CEA）水平，分析其与HE4表达的相关性。结果HE4在肺癌患者中阳性率（42．0％）低于cyfra21-1（55．0％）、CEA（50．0％），高于NSE（27．0％）。肺癌组HE4水平明显高于肺良性疾病组，差异有统计学意义（P〈0．05），肺癌患者血清HE4水平与NSE、eyfra21—1呈正相关（r=0．478，r=0．300，P均〈0．05），与CEA无相关性（r=0．133．P=0．311）。HE4、NSE、eyfra21-1联合检测可提高肺癌诊断的灵敏度（78．0％）和特异性（86．0％）。结论HE4是有价值的肺癌诊断标志物，与NSE、cyfra21—1联合检测有助于提高肺癌的检出率。Objective To study the significance of human epididymis protein 4 （HE4） in the lung cancer diagnosis. Methods HE4 serum levels in 60 patients of lung cancer and 60 patients of benign lung disease were detected by enzyme-linked immunosorbent assay. Analyzing the expression differences between the two groups. The serum levels of carcinoembryonic antigen （CEA）, neuron-specific enolase （NSE）, cytok- eratin 19 fragment （cyfra21-1） were determined by electrochemiluminescence assay. The correlation of these indexes with HE4 were analyzed. Results The positive rate of HE4 in lung cancer patients was 42.0%, which lower than cyfra21-1 （55.0%） and CEA（50.0%）, higher than NSE（27.0%）. The HE4 serum level was significantly higher in lung cancer group than that in benign lung disease group, and the difference had statistical significance （P〈 0.05）. HE4 had positive correlation with NSE and cyfra21-1 in lung cancer patients （r= 0.478, r= 0.300,Pall〈 0.05）, while there was no correlation with CEA （r= 0. 133 ,P= 0.311 ）. The sensibility （78.0%） and specificity （86.0%） were improved when HE4, NSE and cyfra21-1 were combinational detected. Conclusion HE4 is a valuable diagnostic marker for lung cancer and it will improve the detection rate of lung cancer combined with the tumor markers of NSE and cyfra21-1.
