临床药师参与肺癌术后辅助化疗的临床实践与体会

摘　　要：目的分析抗肿瘤药物在临床应用中存在的不规范现象,提出合理性建议,促进抗肿瘤药物的合理应用。方法通过1例肺癌患者术后辅助化疗方案选择不规范,致发生周围神经疾病的案例,临床药师参与整个治疗过程,提出用药建议,并实施全程药学监护。结果术后辅助化疗中抗肿瘤药物使用存在不规范现象,包括:使用推荐方案的药物,但改变用法用量、用药时间;任意"改良"推荐方案;采用非推荐的方案和药物;术后辅助化疗存在治疗过度或治疗不足等。结论制定《抗肿瘤药物临床使用管理办法》,对抗肿瘤药物实行分级管理,临床药师参与抗肿瘤药物的临床应用,提高抗肿瘤药物合理使用水平。Objective To analyze nonstandard phenomenon of the antitumor drugs in the clinical application, and raise a proposal to pro-mote the rational application of antitumor drugs. Methods Clinical pharmacist participated in adjuvant postoperative chemotherapy after resection of lung cancer which induced peripheral disease, put forward some recommendations and took full implementation of pharmaceu-tical care. Resutls There were some nonstandards in the clinical use of antitumor drugs in the Department of Oncology, for example, u-sing recommended drugs but changing the dose and time, changing the chemotherapy scheme in random, using the drugs which is not rec-ommended,treating excessively and insufficiently. Conclusion We should draw up Management Regulations for Clinical use of Antitu-mor, and conduct graded management to improve the rational application of antitumor drugs.
