妊娠期白血病13例并文献分析

摘　　要：目的:研究妊娠期白血病的诊断、治疗及预后。方法:回顾性分析13例妊娠期并发白血病患者的临床资料,并复习相关文献。结果:13例患者中,孕早期5例,孕中期2例,孕晚期6例。孕早期5例中,人流3例,药流1例,自然流产1例;孕中期2例均引产;孕晚期6例中,剖宫产5例,顺产1例。13例中,妊娠期间化疗者2例。结论:孕早期、孕中期白血病,多选择先终止妊娠后治疗白血病。孕晚期白血病可选择化疗,化疗药物的选择及生产时机的确定很重要,或先保胎近足月后行剖宫产或正常生产,再予以化疗。Objective： To evaluate and review the diagnosis, treatment and prognosis of pregnancy associated leukemia. Method：We retrospectively analyzed the clinical characteristics of thirteen cases of pregnancy associated leukemia. Result： The number of patients diagnosed with leukemia during the first, second and third trimester was five, two and six, respectively. For the five cases in the first trimester, one patient had therapeutic abortion, three patients had manual abortion and one patient had spontaneous abortion. The two patients in the second trimester had been induced labour. For the six cases in the third trimester, five patients underwent caesarean section and one patient delivered via vagina. Of all the patients mentioned above, two cases were treated with chemotherapy. Con- clusion： Patients with leukemia during the first and second trimester should be offered with therapeutic abortion and then chemotherapy for leukemia can be started. Chemotherapy may be selected for patients with leukemia diag- nosed in the third trimester, but the choice of drug and timing of delivery should be carefully evaluated. Alterna- tively,the fetus could be delivered at full term before the initiation of chemotherapy.
