单鼻孔蝶窦入路垂体腺瘤切除术后并发症的分析

摘　　要：目的探讨单鼻孔蝶窦入路垂体腺瘤术后并发症及其防治措施。方法回顾分析我科132例单鼻孔蝶窦入路垂体腺瘤切除术后并发症情况。结果本组病例术后发生脑脊液鼻漏5例,尿崩症14例,视觉障碍1例,垂体功能不足2例,蝶窦感染1例,外展神经麻痹1例,有2例患者出现以上两种或两种以上并发症,无昏迷及死亡病例。大部病人都经保守治疗而愈。结论熟知局部的相关解剖,不断提高手术技巧,重视术后处理,有助于减少并发症,提高临床治疗效果。Objective To evaluate the complications of trasnaso-sphenoidal microsurgery for pituitary adenomas, and discuss the problem of effective prevention and management. Methods 132 trasnaso-sphenoidal microsurgery for pituitary adenomas were reviewed retrospectively to evaluate. complications. Results After the operation, there were 5 CSF rhinorrhea patients;14 diabetes insipidus patients; 1 visual disturbance patient;2 supsecreting patients; 1 sphenoidal infection patient; 1 abduct nerve paralysis patient; there were 2 patients with 2 or various complications, and no dead patients. The majority had been cured by the non-operation. Conclusion The master of the local microdisscetion, adequate management after operation and the proficient operation skill can decrease the rate of complication.
