房水引流管植入术治疗难治性青光眼术后并发症分析

摘　　要：目的 :评价房水引流管植入术治疗难治性青光眼的疗效 ,探讨其并发症及预防。方法 :对 78例难治性青光眼施行房水引流管植入术 ,术后随访 3～ 36个月。结果 :眼压由术前 3.4～ 8.12 (5 .6 7± 1.32 ) k Pa下降至 0 .40～ 3.6 7(2 .45± 0 .98) k Pa,手术成功率为 82 %(6 4/ 78) ,术后视力提高或保持不变 84.6 %(6 6 / 78)。并发症包括前房出血、浅前房、脉络膜脱离、引流管内口堵塞、引流管外露、导管退缩到房角或脱出前房、球结膜纤维包囊形成、恶性青光眼。结论 :尽管房水引流管植入术是治疗难治性青光眼的一种有效方法 ,但存在一些并发症应引起医生的重视。Objective:To evaluate the efficacy of the aqueous humor drainage device implantation for refractory glaucoma and to discuss its postoperative complications and management.Methods:78 cases of refractory glaucoma underwent aqueous humor drainage device implantation,and followed up for 3 to 36 months.Results:The intraocular pressure(IOP)of the cases decreased from 3 4～8 12(5 67±1 32)kPa pre operatively to 0 40～3 67(2 45±0 98)kPa post operatively.The success rate was 82%(64/78)The postoperative visual acuity improved or remained in 84 6%(66/78).The complications included hyphema,shallow anterior chamber,choroidal detachment,obstruction of tube tip,tube exposure,tube retraction to anterior chamber angle of leaving away from anterior chamber,fibrous tissue encapsulation of explant and malignant glaucoma.Conclusion:Aqueous humor drainage device implantation is an effective method in management of refractory glaucoma in spite of some complication.
