早期康复训练对改善急性脑梗死偏瘫患者肌张力及神经功能缺损评分的影响

摘　　要：目的探讨早期康复训练对改善急性脑梗死偏瘫患者肌张力及神经功能缺损评分的影响。方法回顾性分析2010年1月～2013年1月在我院就诊的80例急性脑梗死偏瘫患者的临床资料。根据治疗方法不同将上述入选患者随机分为康复组和对照组各40例。比较两组治疗后的疗效及两组上肢、下肢肌张力治疗后的改善情况以及两组患者治疗前后神经功能缺损评分的变化情况。结果康复组治疗后的总有效率达92.5%,显著高于对照组的总有效率,差异存在显著性(P<0.05)。治疗1个月后,上肢:康复组的肌张力以0级为主,对照组以2级为主,康复组的肌张力均明显低于对照组,差异存在显著性(P<0.05);下肢:康复组的肌张力以0级及1级为主,对照组以1+级及2级为主,康复组的肌张力均明显低于对照组,差异存在显著性(P<0.05)。两组患者治疗21天后的神经功能缺损评分分别较治疗前明显降低(P<0.05),治疗后康复组降低幅度较对照组明显(P<0.05)。结论早期康复训练有利于促进急性脑梗死患者神经功能的恢复,使患肢功能及肌张力得到最大程度改善,提高患者的生活质量。Objective To investigate the effect of early rehabilitation training to improve muscle tone in hemiplegic patients with acute cerebral infarction and the impact of neurological function. Methods Eighty cases of acute cerebral hemiplegia patients were randomly divided into rehabilitation group and the control group of 40 cases. After treatment, the efficacy and two upper and lower limbs limb strength improved after treatment as well as two groups of patients before and after treatment of neurological function changes were compared. Results After treatment, the total effective rate of rehabilitation group was 92.5% ,was significantly higher than the control group（P〈0.05 ）. After 1 months of treatment, upper limb： muscle tension rehabilitation group dominated by 0, the control group in two main muscle ten- sion rehabilitation group were significantly lower than the control group （P〈0.05）; leg： rehabilitation group of muscle tension to 0 and a main control group 1＋ grade and two main muscle tension rehabilitation group were significantly lower than the control group （P〈0.05）. Two groups of patients after 21 days of neurological deficit scores were compared with pretreatment,significantly reduced after treatment （P〈0.05）, rehabilitation group reduced lower than the control group （P〈0.05）. Conclusion Early rehabilitation of patients with acute cerebral infarction and promoting recovery of neurological function, so that the greatest degree of improvement in limb function and muscle tension, improve the patient＇s quality of life.
