奥扎格雷钠与巴曲酶联用治疗脑梗死后脑水肿疗效观察

摘　　要：目的探讨奥扎格雷钠与巴曲酶合用对脑梗死后脑水肿的疗效和机理及合用的安全性。方法符合入选标准的80例患者随机分组,其中治疗组44例,对照组36例。两组患者均采用常规治疗方法,治疗组加用奥扎格雷钠、巴曲酶治疗,对照组给予丹红、疏血通静滴。采用BORN-BE无创脑水肿动态监护仪测定双侧大脑半球阻抗值,同时对脑梗死发生后2,7,14d后的梗塞病灶周围水肿带面积及血液中的基质金属蛋白酶-9(MMP-9)的表达情况进行监测。结果治疗组梗塞病灶周围水肿带面积较对照组减少(P<0.01),治疗组患侧大脑半球阻抗值低于对照组(P<0.05),治疗组患者血浆中MMP-9含量在不同时段均低于对照组(P<0.05)。结论奥扎格雷钠与巴曲酶合用可有效的减少脑梗死患者脑水肿的发生,安全性较好。Objective To investigate the effect of ozagrel sodium combined with batroxobin on cerebral infarction patients complicated with cerebral edema. Method The 80 cerebral infarction patients complicated with cerebral edema meeting the inclusion criteria were randomly divided into treatment group （44 cases） and control group （36 eases）. Two groups of patients were treated by routine therapy and sodium ozagrel and batroxobin were added to cases in treatment group,while those in the control group was given Danhong injection, Shuxuetong injection intravenous drip.The bilateral cerebral hemisphere impedance value and matrix metalloproteinase-9 （MMP-9） expression in the blood and the peripheral edema area were monitored 2,7,14 days after cerebral infarction occurred. Results The edema area around infarction in the treatment group reduced than that in the control group （P〈0.01）,and the cerebral hemisphere impedance value of the treatment group was lower than that of the control group （P〈0.05）, and the content of MMP-9 in plasma of patients of treatment group at different time was lower than that of the control group （P〈0.05）. Conclusion Ozagrel sodium combined with batroxobin can effectively reduce the edema in patients with cerebral infarction with high safety.
