原发于腮腺内的面神经鞘瘤临床特点及诊治分析

摘　　要：目的分析原发于腮腺内的面神经鞘瘤的临床特点、诊断及治疗方法。方法术前对拟行腮腺浅叶及肿瘤切除术的患者进行必要的检查,术中取组织冰冻活检。手术注意沿神经走向剥离肿瘤,避免过度牵拉挤压。结果 6例患者经临床检查均确诊为腮腺内面神经鞘瘤,CT、MRI提示腮腺区占位性病变,无骨质破坏,经手术治疗,面神经功能均恢复正常。结论原发于腮腺内的面神经鞘瘤临床较少见,易造成误诊,临床应注意仔细检查辨别;术中轻柔操作,尽量减少对面神经的牵拉,有助于患者面神经功能的恢复。Objective To analyze the clinical characteristics, diagnosis and treatment method of primary facial nerve schwannoma in the parotid gland. Methods Preoperative, patients with superficial lobe parotid gland and tumor resection were given necessary preoperative examinations, and were given tissue frozen biopsy examination intraoperative. The operation should along the nerve to strip the tumor, and avoid excessive force extrusion. Results Six patients were diagnosed of parotid gland facial nerve schwannomas by clinical examination, CT and MRI prompted parotid occupying lesions, no bone destruction, the facial nerve function in patients with surgical treatment was ba^k to normal. Conclusion Primary facial nerve sehwannomas in the parotid gland is uncommon clinically and easily misdiagnosis, clinical attention should be paid to discern carefully check. Intraoperative gentle operation and trying to reduce the pull across the facial nerve were helpful to the facial nerve function re- covery.
