摘 要:目的探讨金属蛋白酶-9(MMP-9)和血管内皮生长因子(VEGF)在宫颈上皮内瘤样病变(CIN)和宫颈鳞癌中的表达及其相关性。方法选择手术切除的宫颈鳞癌组织标本47份作为肿瘤组,宫颈上皮内瘤样病变(CIN)标本35份作为CIN组,正常宫颈组织(NCE)21份作为NCE组,采用免疫组化SP法检测各组MMP-9和VECF的表达和微血管密度(MVD)(CD34标记)。结果肿瘤组、CIN组中MMP-9和VEGF阳性率和MVD值均明显高于NCE组(P<0.05),肿瘤组明显高于CIN组(P<0.05);MMP-9和VEGF阳性表达均与肿瘤组的临床分期、组织分化程度、有无淋巴结转移相关,并且在宫颈鳞癌组织中两者表达呈正相关;肿瘤组中MMP-9和VEG;F阳性者的MVD值明显高于其阴性者(P<0.05),MMP-9和VEGF的表达与MVD值存在显著性差异(P<0.05)。结论 MMP-9和VEGF在宫颈鳞癌发生、发展中可能起重要作用,MMP-9和VEGF及MVD值与宫颈鳞癌的浸润转移密切相关,可作为预后的判断指标。Objective It is to explore the expression of MMP -9 and VEGF in CIN and squamous carcinoma of cervix and its relationship. Methods The expression of MMP -9 and VEGF, microvessel density(MVD) labeled by CD43 in 21 cases of normal cervical epithelium (NCE) ( NCE group), 35 cases of cervical intraepithelial neoplasm ( CIN ) ( CIN group ) and 47cases of squamous carcinoma of cervix (tumor group) were detected by immunohistochemistry SP method. Results The positive ex- pression rates of MMP - 9 and VEGF and MVD in tumor group and CIN group were significantly higher than that in NCE group ( P 〈 0. 05 ). The positive expression rates in tumor group was were higher than that in CIN group( P 〈0. 05 ) Both of their ex- pressions were related with lymph node metastasis, clinical stage and pathological staging( P 〈 0.05 ). MVD of the patients with positive MMP - 9 and VEGF was obviously higher than that with negative MMP - 9 and VEGF in tumor patients ( P 〈 0.05 ) , there was significant difference in MVD And the expressions in squamous carcinoma were positively related with each other. The expressions of MMP -9 and VEGF were positively correlated with MVD (P 〈 0.05 ). Conclusion The positive ex- pression of MMP - 9 and VEGF may play an important role during the development of tumor, MMP - 9 and VEGF and MVD are closely related with the invasion and metastasis of tumor, which can be used as the indexes for the prognosis of tumor.
