摘　　要：目的:研究血清EPCA-2(early prostate cancer antigen-2,EPCA-2)与PSA在前列腺癌术前术后变化情况。方法:收集良性前列腺增生(benign prostatic hyperplasia,BPH)患者40例,前列腺癌患者25例,用酶联免疫吸附测定法(enzymelinked immunosorbent assay,ELISA)分别检测术前术后血清EPCA-2和前列腺特异性抗原(prostate-specific antigen,PSA),所得数据的分析和处理采用SPSS13.0统计学软件进行。结果:(1)前列腺癌组患者术前血清中的EPCA-2和PSA水平均明显增高,与BPH组比较差异具有统计学意义(P<0.01);(2)前列腺癌组患者术后血清中的EPCA-2明显增高,与术前比较差异具有统计学意义;BPH组患者术后血清中的PSA增高后降低,与术前比较差异具有统计学意义。结论:(1)EPCA-2可以用于前列腺癌的筛查。(2)前列腺癌组与BPH组患者术前血清中的EPCA-2和PSA水平均明显增高。(3)前列腺癌组患者术后血清中的EPCA-2明显增高术前;BPH组患者术后血清中的PSA增高后降低。(4)EPCA-2可作为前列腺癌的生物标记物,用于前列腺癌的诊断,为前列腺癌预后的判断提供一个检测项目。Objective： To investigate the clinical significance of serum EPCA-2 in the diagnosis of prostate cancer.Methods： Serum EPCA-2 and PSA of 40 patients with BPH and 22 patients with prostate cancer were tested respectively by ELISA,and the data was dealt with statistics software SPSS13.0.Results：（1）Compared with patients with BPH,the levels of serum EPCA-2 and PSA in patients with prostate cancer increased obviously,the difference between them was statistically significant（P0.01）.（2） Using a cutoff of 30ng/mL,EPCA-2 has a specificity of 91.2% and a sensitivity of 95.4% in separating men with BPH from those with prostate cancer.The specificity for PSA at a cutoff of 4.0ngl/mL was 61.8%,and the sensitivity was 81.8%.Conclusion：（1）EPCA-2 can serve as a biomarker for diagnosis of prostate cancer;（2）the preoperative levels of serum EPCA and PSA increased obviously in both prostate cancer group and BPH group;（3）the postoperative level of serum EPCA-2 increased obviously higher than the one of preoperative;（4） EPCA-2 can be served as a bio-marker for the diagnosis of prostate cancer,offering a detecting item for the judgement of prostate cancer prognosis.
