血常规检验在希森美康和雅培检测系统的结果分析

摘　　要：目的探索如何解决雅培血球质控品因有效期短、国内无经营许可证导致境外采购,经常供应不及时造成没有室内质控怎么保证血常规检验结果准确可靠的问题。方法将58份新鲜标本同时在希森美康KX-21和雅培CELL-DYN 3700血球仪上检测,比较7个主要指标的均值、相关性。结果白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(Hb)、血小板计数(PLT)、血细胞比容(Hct)、淋巴细胞百分比(LYM%)、中性粒细胞百分比(NEU%)等指标在希森美康和雅培血球仪上检测均值分别为WBC(10.36±11.70)、(11.47±12.98)×109/L,RBC(3.46±0.87)×1012/L、(3.56±0.89)×1012/L,Hb(94.8±19.8)、(95.5±19.9)g/L,PLT(174±170)×109/L、(173±161)×109/L,Hct(30.5±6.4)%、(27.6±5.9)%,LYM%(16.4±12.8)%、(16.5±12.4)%,NEU%(73.7±18.3)%、(74.2±17.8)%,相关性分别为1,1,1,1,0.98,0.84,0.89,两个系统检测相比较,Hct差异有统计学意义(P<0.01),其余6项差异无统计学意义(P>0.05)。结论日常工作中,在保证希森美康血球仪在控的前提下,每天抽取3个标本(最好包含高、中、低值)在两个系统上比对,同时调整Hct参数为1.11,可以保证用雅培血球仪检测结果的准确性。Objective To explore how to ensure the accuracy and reliability of Abbott CELL-DYN 3700for routine blood test.Methods 58fresh specimens were detected both in Sysmex KX-21and Abbott CELL-DYN 3700 at the same time,mean values and correlation of the seven major indicators were compared.Results The values of white blood cell count（WBC）,red blood cell count（RBC）,hemoglobin（Hb）,blood platelet count（PLT）,hematokrit（Hct）,lymphocyte percentage（LYM%）and neutrophils percentage（NEU%）,detected by Sysmex KX-21 were（10.36±11.70）×109/L,（3.46±0.87）×1012/L,（94.8±19.8）g/L,（174±170）×109/L,（30.5±6.4）%,（16.4±12.8）%and（73.7±18.3）%,those detected by Abbott CELL-DYN 3700were（11.47±12.98）×109/L,（3.56±0.89）×1012/L,（95.5±19.9）g/L,（173±161）×109/L,（27.6±5.9）%,（16.5±12.4）% and（74.2± 17.8）%,and the correlation coefficients were 1,1,1,1,0.98,0.84and 0.89.Except Hct,detected results of the other six indicators were not statistically different between the two analyzers（P0.05）.Conclusion In daily work,under the premise that quality control of Sysmex KX-21is fine,accuracy of the results detected by Abbott CELLDYN 3700could be assured by comparative analysis of three specimens,including high,medium and low levels,on the two systems and by adjustment of Hct parameter to 1.11at the same time.
