早期动脉乳酸清除率对危重病患者预后效果评价分析

摘　　要：目的探讨动脉血乳酸及6h乳酸清除率的监测对危重病患者预后效果评价分析。方法选取101例危重病患者,按照入院后APACHEⅡ评分不同,将患者分为：≤10分（A组）,〉10-20分（B组）,〉20-30分（C组）,〉30分（D组）4个组,以及对存活组和死亡组进行6h乳酸清除率、APACHEⅡ评分等指标进行比较,并分析其与预后效果的关系。结果 A组和B组患者6h乳酸清除率和病死率比较差异无统计学意义（P〉0.05）,C组患者6h乳酸清除率和病死率低于B组（P〈0.05）;D组患者6h乳酸清除率和病死率低于C组（P〈0.05）,且6h动脉血乳酸清除率与APACHEⅡ评分呈显著性负相关（r=0.695,P〈0.01）。结论 6h动脉血乳酸清除率可对危重病患者病情进行监测和早期干预,可作为危重患者预后进行效果评价的指标。Objective To Investigate and monitor arterial lactate and lactate clearance rate in six hours for prognosis evaluation and analysis in critically ill patients.Methods 101cases of critically ill patients were divided into：≤10points（A group）,10-20 points（B group）,20-30points（C group）,30points（D group）four groups according to APACHE Ⅱscore after admission and the survival group and death group were compared in lactate clearance rate in six hours,APACHEⅡscore to analyze the relationship with prognosis.Results the patients in group A and group B for lactate clearance rate in six hours and mortality was no significant difference（P0.05）,the patients in group C were lower than group B but groups were significant differences（P 0.05）;the patients in group D were lower than group C（P0.05）,and lactate clearance rate in six hours and APACHE Ⅱscore was significantly negatively correlated（r=0.695,P0.01）.Conclusion Arterial blood lactate clearance rate in six hours can be used for monitoring,early intervention in critically ill patients and evaluation indicators for prognosis of critically ill patients.
