急性胰腺炎患者血清降钙素原、超敏C反应蛋白、白细胞介素6水平变化及临床意义

摘　　要：目的探讨急性胰腺炎患者血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)及白细胞介素-6(IL-6)水平变化及临床意义。方法选择82例急性胰腺炎患者,按其病情程度分为轻症急性胰腺炎(MAP)组46例、重症急性胰腺炎(SAP)组36例,分别于入院第1天、第3天、第7天对其血清PCT、hs-CRP、IL-6等指标水平进行检测,并与正常对照组40例健康受试者进行比较分析。结果入院第1天,MAP组、SAP组患者血清PCT、hs-CRP、IL-6水平均显著高于正常对照组,尤其以SAP组为甚,差异均具有统计学意义(P<0.05);入院第3天及第7天,SAP组与MAP组患者血清PCT、hs-CRP、IL-6水平均呈下降趋势,但MAP组、SAP组患者血清PCT、hs-CRP、IL-6水平均仍显著高于正常对照组,仍尤以SAP组为甚,差异有统计学意义(P<0.05);且急性胰腺炎患者入院时血清PCT、hs-CRP、IL-6等指标水平两两之间均呈正性显著相关(P<0.05)。结论监测急性胰腺炎患者血清PCT、hs-CRP、IL-6水平变化,对疾病的早期诊断、病情判断和预后评估均具有重要的临床意义。Objective To explore the clinical significance of detecting the serum levels of procalcitonin（PCT）,high-sensitivity Creactive protein（hs-CRP）,interleukin 6（IL-6）in the patients with acute pancreatitis（AP）.Methods 82cases with AP were selected and divided into severe acute pancreatitis（SAP）group（n=36）and mild acute pancreatitis（MAP）group（n=46）.The serum levels of PCT,hs-CRP,IL-6were detected on the 1st,3rd,and 7th day in all patients,and compared with those of 40healthy subjects in the control group.Results The serum levels of TNF-α,IL-6,hs-CRP in the MAP group and the SAP group on the 1st day were significantly higher than those in the control group（P0.05）.The serum levels of PCT,hs-CRP,IL-6in the two groups on the 3rd and the 7th day were decreased,but the serum levels of PCT,hs-CRP,IL-6were significantly higher than those in the control group（P0.05）.The serum levels of PCT,hs-CRP were actively correlated with IL-6in the patients with AP（P0.05）.Conclusion The detection of serum levels of PCT,hs-CRP,IL-6play an important role in early diagnosis of the disease,the condition judgment and the basis for prognostic evaluation of patients with AP.
