VEGF与uPA系统在胃癌病情发展的相关性研究

摘　　要：目的研究血管内皮生长因子(VEGF)和尿激酶型纤溶酶原激活物(uPA)系统与胃癌病情发展的相关性,为提高胃癌的临床疗效提供理论依据。方法选取该院胃肠外科行胃癌手术治疗,病理检查确定为胃癌的158份标本为观察组,选取同期正常的胃黏膜组织标本48份为对照组,检测对比两组标本的VEGF、尿激酶型纤溶酶原激活物以及尿激酶型纤溶酶原激活物受体的表达情况,并和胃癌的病理特征进行相关性分析。结果观察组三项指标的阳性率均明显高于对照组(P<0.05);而肿瘤的浸润程度越高、分化程度越低、伴淋巴转移以及临床病理分期(TNM)越高,其指标的阳性率越高(P<0.05)。观察组标本的VEGF和尿激酶型纤溶酶原激活物表达呈正相关关系(r=0.413,P=0.008);尿激酶型纤溶酶原激活物和尿激酶型纤溶酶原激活物受体表达呈正相关关系(r=0.404,P=0.012)。结论 VEGF与uPA系统的表达与胃癌病情的发展关系密切,其对胃癌的诊断以及预后的评估均有参考价值。同时,也能够作为新的治疗靶点,为临床治疗胃癌提供一条新的策略。Objective To study the correlation of vascular endothelial growth factor（VEGF）and urokinase plasminogen activator（uPA）system with the progression of gastric cancer,and provide a theoretical basis for treating gastric cancer.Methods Selected 158specimens of gastric cancer identified by pathological examination,were selected as observation group,and 48normal gastric mucosa specimens as control group.The expression of VEGF,uPA and uPAR were compared.And analyzed the relationship between pathological features of gastric cancer and their expressions.Results The positive rate of the three indicators of the observation group were significantly higher than the control group（P0.05）.And the degree of tumor infiltration of the higher,less differentiated,with lymphatic metastasis and TNM stage of higher and higher,the positive rates of the index was higher（P0.05）.The VEGF and the uPA in the observation group was positively correlated（r=0.413,P=0.008）,and the expression of uPA and uPAR was positively correlated（r=0.404,P=0.012）.Conclusion VEGF and uPA system can significant impact the progression of gastric cancer.The detection of the indicators can provide an useful clue for gastric cancer diagnosis and prognostic evaluation.It also can be used as a new therapeutic targets and provided a new strategy for the treatment of gastric cancer.
