摘 要:目的探讨促红细胞生成素(EPO)在非小细胞肺癌(NSCLC)组织中的表达及其与新生血管形成的关系。方法选择我院2010年1—9月收治的NSCLC患者58例,收集其术后肺癌组织蜡块作为观察组,非肿瘤性肺组织作为癌旁对照组。选择我院同期收治的肺良性病变患者45例,收集其术后良性病变肺组织蜡块作为良性病变对照组。采用免疫组织化学法检测EPO的表达,CD34染色法观察新生微血管形成情况,计算微血管密度(MVD)。结果 3组EPO阳性表达率比较,差异有统计学意义(P=0.000),且观察组高于癌旁对照组(P=0.000)和良性病变对照组(P=0.000)。不同性别、年龄、pTNM分期、病理类型、分化程度及是否吸烟、有无淋巴结转移的NSCLC患者EPO表达阳性率比较,差异均无统计学意义(P>0.05)。观察组中EPO阳性表达者MVD大于EPO阴性表达者,差异有统计学意义(P=0.001)。结论 EPO在NSCLC组织中高表达,可将EPO阳性表达作为NSCLC的辅助性诊断指标。EPO阳性表达与MVD有关,在NSCLC治疗过程中应用EPO治疗肿瘤相关的贫血时要考虑患者的获益及风险。Objective To detect the expression of erythropoietin (EPO) in non - small cell lung cancer (NSCLC) and its correlation with angiogenesis. Methods A total of 58 patients with NSCLC admitted to our hospital between January and September 2010 were included in the study. The paraffin - embedded lung cancer tissues and non - neoplastic lung tissues of the subjects were assigned as the observation group and adjacent group, respectively. The paraffin - embedded tissues of benign lung lesions from 45 patients with benign lung lesions admitted to our hospital during the same period were assigned as the benign lesion group. The expression of EPO were detected using immunohistochemistry. The CD34 staining method was used to observe anglo- genesis and microvessel density (MVD) was calculated. Results The positive rate of EPO expression in the observation group was significantly higher than that in the adjacent group ( P = 0. 000) and the benign lesion group ( P = 0. 000). The results showed that the expression of EPO was not related to age, sex, pTNM stage, pathological type, differentiation state, smoking condition, and lymph node metastasis (P 〉 O. 05). The EPO - positive patients had higher MVD than EPO - negative patients in the observation group (P = 0. 001 ). Conclusion High expression of EPO in NSCLC patients suggests that positive EPO ex- pression may be used as an auxiliary diagnostic index for NSCLC. Due to the correlation between positive EPO expression and MVD, the clinician should weigh the benefits and risks of patients when using EPO to combat anemia in the treatment of NSCLC.
