足三里注射对腹腔镜术后胃肠动力的影响

摘　　要：目的探讨足三里注射胃复安对腹腔镜胆囊切除术后患者胃肠动力的影响。方法将2012年1月～2012年8月行腹腔镜胆囊切除术的患者120例，按随机数字表法分为治疗组与对照组。两组均予以常规治疗，治疗组在常规治疗基础上辅以胃复安穴位注射。比较两组患者术后第1次肛门排气时间、术后腹胀发生率；采用放射免疫法检测术前、术后第1、2、3天血清胃动素水平。结果两组患者手术时间、术中出血比较差异无统计学意义（P〉0．05）。治疗组第1次肛门排便时间较对照组明显缩短（P〈0．05）；腹胀发生率明显低于对照组（P〈0．05）；术后血清胃动素水平下降明显低于对照组（P〈0．01）。结论腹腔镜胆囊切除术后辅以足三里注射胃复安治疗可缩短患者胃肠功能恢复时间，有效防止术后腹胀发生。Objective To explore the impacts of the injection of metoclopramide at Zusanli（ST36） on gastrointestinal motility after laparoscopic cholecystectomy. Methods One hundred and twenty patients of laparoscopic cholecystectomy were collected from January 2012 to August 2012 and were randomized into a treatment group and a control group. The conventional treatment was applied in the two groups. In the treat-ment group, on the basic treatment, the acupoint injection of metoclopramide was assisted. The time of the first exsufflation and the morbidity of abdominal distention after operation were compared between the two groups. The radioimmunoassay（RIA） was adopted to detect the level of serum motilin before treatment and in the 1 ~t, 2~a and 3rd days after treatment separately. Results The differences in operation time and hemorrhage during operation were not significant between the two groups（ P 〉 0.05 ）. The first exsufflation in the treatment group happened much earlier than that in the control group（ P 〈 0.05 ）. The morbidity of abdominal distention was lower obviously than that in the control group（ P 〈 0.05 ）. The postoperative serum motilin level was lower ob- viously than that in the control group （ P 〈 0.01 ）. Conclusion The laparoseopic cholecysteetomy assisted with the injection of metoclopramide at Zusanli（ST36） shortens the recovery time of gastrointestinal function and effectively prevents from abdominal distention after operation.
