摘　　要：目的研究血管内皮生长因子(VEGF)在初发和复发鼻息肉组织中表达的不同,探讨它在初发和复发鼻息肉发病机制中的作用。方法选择初发鼻息肉组40例,复发鼻息肉组40例,正常下鼻甲黏膜对照组20例,下鼻甲黏膜组织标本来自鼻中隔偏曲伴下鼻甲肥大的患者。采用免疫组织化学方法观察VEGF在初发鼻息肉组和复发鼻息肉组和正常下鼻甲黏膜中的表达,比较VEGF在初发和复发鼻息肉2组间和2组组内不同性别中表达的差异。结果 VEGF在初发鼻息肉和复发鼻息肉组织中主要表达于基底膜下的血管内皮细胞和炎性细胞,2组间比较差异有统计学意义(P=0.038)。在初发鼻息肉中,不同性别间比较差异无统计学意义(P=0.38)。在复发鼻息肉中,不同性别间比较差异无统计学意义(P=0.27)。在正常下鼻甲黏膜中无明显表达。结论 VEGF对初发鼻息肉和复发鼻息肉发病过程中组织水肿的产生和血管的密集增生有很重要的作用。VEGF通过在鼻息肉组织中的大量表达,使息肉组织中的血管增多、通透性增强和炎性细胞浸润,组织水肿加重,加速了鼻息肉的形成或复发。Objective To investigate the expression of vascular endothelial growth factor（VEGF）in primary and recurrent nasal polyps and to explore the probable function of it in nasal polyps pathogeneses.Methods A study was conducted on 100 cases,40 samples of primary nasal polyps,40 samples of recurrent nasal polyps,and 20 samples of inferior turbinate mucosa.Samples of inferior turbinate mucosa,as normal control,were obtained from the patients undergoing septal derivation with hypertrophic inferior turbinate.Immunohistochemistry was used to observe the espression of VEGF in there groups.Results VEGF was located mainly in the inflammatory cells under the basilar membrane and vascular endothelial cells in primary and recurrent nasal polyps.The comparison in recurrent nasal polyps was significantly higher than in primary nasal polyps（P=0.038）.The comparison in primary and recurrent nasal polyps between the gender was not different.There is no obvious expression in inferior turbinate mucosa tissues.Conclusion VEGF plays very important roles in the production of tissue edema and vascular accrementition in primary and recurrent nasal polyps.The generous expression of VEGF in nasal polyps makes vessels increase,permeability enhance and inflammatory cells infiltrate.These aggravate tissue edema and accelerate the formation or recurrence of nasal polyps.
