摘 要:目的评估男性吸烟者肺功能下降程度与腹围之间的关系。方法纳入我院每年固定体检者,2009年4—6月(队列A)和2011年4—6月(队列B)对体检者进行肺功能检测、腹围测定、相关实验室指标检测,对队列B的体检者按是否存在代谢综合征(MS)分层研究。对队列A和队列B的相关指标进行统计学分析。结果共纳入126名符合标准的体检者,队列B中共有25例MS患者。队列A与队列B相比,用力肺活量占预计值百分比(FVC%pre)、第一秒用力呼气容积/用力肺活量(FEV1/FVC)差异有统计学意义(P<0.05);FEV1/FVC<0.7的百分比队列B高于队列A,差异有统计学意义(P<0.05)。以FEV1平均年降低值(ΔFEV1)及FVC平均年降低值(ΔFVC)与各因素做相关分析,发现ΔFEV1仅与腹围(AC)相关(r=-0.39,P=0.02),与其他因素不相关;以ΔFEV1≥1.6%为自变量,ΔFEV1的降低因素为AC〔OR=1.29,95%CI(1.12,1.38),P<0.01〕。与非MS的体检者特征相比,MS组吸烟指数更高;MS组的体质指数的年变化速率为0.89 kg/m2,而非MS组的体质指数变化不大;MS组体检者FEV1出现下降的人数为9人(36.0%),而非MS组的患者FEV1出现下降的人数为16人(15.8%),两组相比差异均有统计学意义(P<0.05)。两组患者的FEV1均逐年下降,但MS组的下降速率更快,降低速率差异有统计学意义(P<0.05)。结论吸烟者的AC和FEV1存在负相关关系。MS患者的肺功能降低更为明显,MS患者FEV1与非MS患者相比下降速率更快。测量AC是一个简单有效的评估男性吸烟者肺功能下降程度的方法。Objective To evaluate the relationship between pulmonary function and abdominal circumference in male smokers.Methods The males who got regular physical check-up in our hospital were recruited as the subjects,Those with their check-up in April-June 2009(cohort A) and those in April-June 2011(cohort B) were given pulmonary function test,abdominal circumference(AC) measurement,detection of related laboratory indexes.The males of cohort B were given stratification research according to their presence or absence of metabolic syndrome(MS).The relevant parameters of cohort A and B were statistically analyzed.Results A total of 126 eligible males were enrolled.In cohort B there were 25 MS patients.The comparison between the two cohorts revealed significant differences in FVC%-pre and FEV1/ FVC(P 0.05),and the percentage of FEV1/ FVC 0.7 was significantly higher in cohort B than in cohort A(P 0.05).The Correlation analysis showed that the average annual reduction of value for FEV1(ΔFEV1) was only correlated with AC(r =-0.39,P =0.02),not associated with other factors.Taking Δ FEV1≥1.6% as independent variable,the reducing factor of ΔFEV1 was AC 〔OR = 1.29,95% CI(1.12,1.38),P 0.01〕.The males with MS had higher smoking index and their annual change rate of body mass index(BMI) was 0.89 kg/m2,but the males with no MS only had al little change of annual change rate of BMI.The number of males whose FEV1 value went down was 9(36.0%) in the MS group,but 16(15.8%) in the non-MS group,the difference was statistically significant(P 0.05).The value of FEV1 went down year by year in both groups,however,it was significantly faster in the MS group than in the non-MS group(P 0.05).Conclusion There is a negative correlation between AC and FEV1 in smokers.The decrease of FEV1 value of pulmonary function is faster in patients with MS than in those with no MS.AC measurement is a simple and effective method for assessing the lung function decline of male smokers.
