摘 要:目的探讨大鼠大脑中动脉缺血再灌注(middle cerebral artery occlusion,MCAO)模型不同时期梗死区周围组织胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)、神经元特异性烯醇化酶(neuron-specific enolase,NSE)、脑源性神经营养因子(brain derived neurotrophic factor,BDNF)、巢蛋白(Nestin)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)的表达变化及其意义。方法采用线栓法制作大鼠MCAO模型,并随机分为模型1、3、7、14、21d组。采用免疫组化观察大鼠模型脑组织GFAP、NSE、BDNF、Nestin、TNF-α的表达。采用改良神经功能缺损严重程度量表(modified neurological severity score,mNSS)对MCAO模型大鼠进行神经功能评价。结果大鼠神经功能评分随MCAO术后时间延长而逐渐下降(F=121.282,P<0.05)。GFAP阳性细胞数量随MCAO术后时间推移呈双向反应,术后1、3、7d组逐渐升高,而至14d组和21d组则又逐步降低,其中7d组高于1d组、3d组(P<0.05)。NSE累积光密度(integrated optical density,IOD)随时间延长逐渐上升,其中1d组小于3d组、7d组、14d组、21d组,21d组大于3d组、7d组(P<0.05)。BDNF IOD值呈双向表达,1d组、3d组、7d组、14d组表达逐步降低,21d表达升高,其中1d组和21d组大于14d组(P<0.05)。Nestin+细胞数量随时间的推移呈双向反应,1d组~7d组逐步升高,随后逐步降低,其中7d组高于1d组、3d组(P<0.05)。MCAO后TNF-α表达无统计学差异(P=0.12)。结论 MCAO在急性期(7d前)受损脑组织产生炎症及抗炎反应明显,急性期后(7d后)以神经生长因子产生并促进神经再生为主。Objective To investigate the expression and trends of glial fibrillary acidic protein (GFAP), neuron-specific enolase (NSE), brain-derived neurotrophic factor (BDNF), Nestin, tumor necrosis factor-a (TNF-a) in tissue surrounding the infarction during the acute and subacute phases after cerebral ischemia-reperfusion injury in rats. Methods Reperfusion model in rats was induced by the intraluminal suture method and randomly divided into model 1 d group, 3 d group, 7 d group, 14 d group, and 21 d group. Immunohistochemistry was used to detect the expressions of GFAP, NSE, BDNF, Nestin, and TNF-a. The mNSS was used to evaluate the neurological function of MCAO rats. Results The mNSS score gradually decreased over time after MCAO, of which the recording on the model 21 d group was significantly less than that on the model 1 d group (P〈0.05). The numbers of GFAP positive cells appeared a two-way response with the passage of time. It gradually increased from model 1 d to 7 d group, gradually decreased at model 14 d and 21 d group. It was significantly higher at model 7 d group than that at model 1 d group and model 3 d group (P〈0. 05). NSE integrated optical density (IOD) increased gradually. It was significantly less at model 1 d group than that at model 3 d group, 7 d group, 14 d group and 21 d group. It was significantly greater at model 21 d group than those at model 3 d group and model 7 d group (P〈0.05). BDNF~ s IOD appeared two-way response, it began to decrease from model 1 d to 14 d group, then increased at day model 21 d group. It was significantly higher at model 1 d and 21 d group than that at model 14 d group (P〈0.05). The amount of Nestin positive ceils presented a two-way response with the passage of time. It rose from model 1 d to 7 d group, then it gradually decreased. It was significantly greater at model 7 d group than that at model 1 d and model 3 d group (P〈0.05).
