摘  要:目的:分析浙江省淳安县肺炎克雷伯菌临床菌株gyrA和parC基因喹诺酮耐药决定区(QRDR)突变类型及与耐药的相关性。方法:检测87株肺炎克雷伯菌临床菌株对3种喹诺酮类药物的最小抑菌浓度(MIC),PCR扩增26株临床菌株的gyrA和parC基因并测序。结果:87株临床菌株对诺氟沙星、环丙沙星和左氧氟沙星的耐药率分别为58.6%、55.2%和51.7%。16株耐药株全部发生gyrA基因S83突变,9株发生parC基因的S80突变,MICs范围16~≥128mg/L,7株未发生parC基因的S80突变,MICs范围4~32mg/L。结论:gyrA和parC基因突变在淋肺炎克雷伯菌对喹诺酮类药物耐药中起重要作用,parC基因S80位突变与耐药程度密切相关。[ Objective] To analyze the relationship between the mutations in the quinolone - resistance - determining- region (QRDR)of gyrA and parC genes and the quinolone resistance in K. pneumoniae isolates. [Method] The MICs of 3 quinolone - drugs were determined. PCR and DNA sequencing were conducted to analyze QRDR associated genes of gyrA and parC in 26 isolates. [Result]The resistance rote of the 87 isolates were 58.6%, 55.2% and 51.7% to norfloxacin, ciprofloxacin and lev- ofloxacm, respectively. In gyrA gene, all 16 resistant isolates had mutation of S83. 9 of 16 resistant isolates had mutalion of S80 with MICs ranging from 16 to ≥ 128mg/L. 7 of 16 resistant isolates were not found mutation of S80 with MICs ranging from 4 to 32. [ Conclusion] Mutations in gyrA and parC genes play an important role in the development of quinolone resistance in K. pneumoniae. Mutation of S80 in parC gene are closely associated with high quinolone - resistance.
