摘  要:目的牙周炎是最常见的口腔疾病之一,可导致牙周支持组织的破坏,最终造成牙齿丧失。组织工程技术在牙周组织修复再生中的应用为牙周炎的治疗开辟了新途径。研究旨在探讨牙龈成纤维细胞(gingival fibroblast,GF)-玉米醇溶蛋白(Zein)-中药双黄补复合体促进犬牙周组织修复再生的作用。方法以2只Beagle犬双侧上颌54|45、下颌743|347共20个牙位作为实验部位,将其配对分为4组,每组5个牙位分别制备骨缺损模型。实验Ⅰ组:翻瓣术+Zein支架植入;实验Ⅱ组:翻瓣术+GF-Zein支架植入;实验Ⅲ组:翻瓣术+GF-Zein-中药双黄补复合体植入;对照组:单纯翻瓣术。于术后3个月进行组织学观察、测量及锥形束CT(cone-beam computed tomography,CBCT)检查。结果实验Ⅰ、Ⅱ、Ⅲ组术后3个月新生牙槽骨高度均高于对照组(P<0.05),其中实验Ⅲ组新骨生成效果更显著(P<0.01);实验Ⅲ组新生牙骨质的测量值比对照组相应值高(P<0.05)。实验Ⅱ组、实验Ⅲ组术区牙槽骨与正常牙槽骨的骨密度差值百分比低于对照组的相应值(P<0.05),实验Ⅰ组与对照组差异无统计学意义(P>0.05)。结论中药双黄补可促进犬牙周骨缺损的修复,有利于牙周组织的再生。Objective Periodontitis, one of the most common oral diseases, may damage the periodontal tissue and lead to tooth loss. The apphcation of tissue engineering technology to periodontal tissue regeneration has paved a new ground for the treatment of periodontitis. This paper is to investigate the effect of gingival fibroblast (GF) -Zein-Chinese medicine Shuanghuangbn (SHB) com- plex on the regeneration of the experimental periodontium. Methods Bone defect models were made with 20 teeth from 2 beagle dogs and divided into 4 groups of equal number: flap surgery control and experimental groups I (flap surgery + Zein), 1I (flap sur- gery + GF-Zein) and m (flap surgery + GF-Zein-SHB). Three months after operation, specimens were obtained from different groups for histological observation and measurement under the microscope and examination by cone-beam CT scanning. Results At 3 months after operation, the newly formed alveolar bone in the experimental groups was significantly higher than that in the control ( P 〈 0.05 ), most significantly in group m ( P 〈 o. Ol ). Cementum regeneration was remarkably higher in group m ( P 〈 o. 05 ) , while the percentage of bone mineral density difference between the normal and operation regions was dramatically lower in groups II and HI than in the control ( P 〈 0.05 ), but with no significant difference between group I and the control group ( P 〉 0.05 ). Conclusion The Chinese medicine Shuanghuangbu can promote the repair of periodontal bone defect and the regeneration of periodontium.
