摘  要:目的:探讨磁性附着体与改良杆卡式附着体两种义齿修复单侧游离缺失对支持组织应力分布的影响。方法:分别以磁性、改良杆卡式两种固位形式的附着体义齿修复下颌单侧游离缺失,用三维光弹应力冻结切片技术测试义齿在垂直力、侧向力加载时对基牙和缺牙区牙槽骨应力分布大小。结果:①两种义齿对基牙牙槽骨的应力比较:磁性附着体垂直力明显大于磁性附着体侧向力(P<0.01),磁性附着体垂直力与改良杆卡式附着体垂直力无差别(P>0.05),磁性附着体侧向力小于改良杆卡式附着体侧向力(P<0.05);改良杆卡式附着体垂直力大于改良杆卡式附着体侧向力(P<0.05)。②两种义齿对缺牙区牙槽骨应力比较:磁性附着体垂直力明显大于磁性附着体侧向力(P<0.01),磁性附着体垂直力与改良杆卡式附着体垂直力无差别(P>0.05),磁性附着体侧向力小于改良杆卡式附着体侧向力(P<0.05);改良杆卡式附着体垂直力与改良杆卡式附着体侧向力无差别(P>0.05)。结论:磁固位附着体与改良杆卡式附着体两种义齿都能达到较好的修复效果,垂直力时两种义齿对观测点的应力分布较均匀,并且两种义齿无差别;侧向力的磁性附着体可发生相对移位形成应力中断,相对于杆卡式附着体能够达到更好的应力分布,可以明显降低基牙周围骨组织的应力。Objective: to investigate the effect of the magnetic attachment and improved rod cassette attachment on the stress of distribution in the supporting tissues in order to repair unilateral free deficiency of support groups. Method: re- spectively by magnetic and improvement rod cassette, two retention form of attachment denture repair mandibular unilateral free missing, with three dimensional photoelastic stress frozen section technology test denture in vertical force,lateral force loading on the abutment teeth area and lack of alveolar bone stress distribution size. Result: (~)Analysis comparison to the two denture base fang of osseous stress: magnetic attachment vertical force is greater than magnetic attachment lateral torce (P 〈0.O1),There are no differences between magnetic attachment vertical torce and improved rod cassette attachment verti- cal force (P 〉0.05) ,magnetic attachment lateral force is less than the improved rod cassette attachment lateral force (P 〈 0.05);improved rod cassette attachment vertical force is greater than improved rod cassette attachment lateral force (P 〈 0.05). 〈~)Analysis comparison to the two denture lacked of dental area alveolar hone stress:magnetic attachment verticalforce is greater than magnetic attachment lateral force (P 〈0.0l),magnetic attachment vertical force and improved rod eas-sette attachment vertical force indifference (P 〉0.05), magnetic attachment lateral force is less than the improved rod cassette attachment lateral force (P 〈0.05) ; There are no differences between improved rod cassette attachment vertical force and im- proved rod cassette attachment lateral force (P 〉0.05).
