MK-1精密附件修复肯氏Ⅱ类牙列缺损病例重衬前后基牙牙周情况变化分析

摘　　要：目的:探讨MK-1精密附着体修复病例半年后重衬对维护基牙牙周组织健康延长义齿使用寿命的临床意义。方法:诊断为KennedyⅡ类牙列缺损,远中游离端基牙健康,拟行MK-1精密附着体义齿修复的患者12例。采用自身前后对照的方法,依照金属烤瓷冠预备要求进行牙体预备,并依次戴入精密附件。附件戴入口内后分别于0、3、6个月,以及6个月时义齿重衬3个月后(戴入义齿第9个月)采集待测基牙龈沟液(GCF)、复查和记录临床相关指标、测定待测基牙龈沟液含量、测定游离端牙槽骨吸收高度,并将各项检查结果记录。经过修复后第3,6,9个月的随访观察,分析各项指标变化。结果:精密附着体义齿修复后不同观察时间的临床检查指标及龈沟液含量变化差异有统计学意义(P<0.05)。结论:MK-1精密附着体修复半年后游离端牙槽骨出现吸收,义齿与黏膜密合度降低,游离端基牙应力增大,出现轻度牙周炎症,经过重衬处理,改善了义齿下沉对基牙造成的损伤性外力,有助于近期基牙牙周组织恢复,最大限度保护基牙,延长义齿使用寿命,值得在临床推行。Objective： To explore MK--1 precision attachment denture case half a year after heavy lining on the maintenance of abutment periodontal tissue health and extend the life of the denture clinical significance. Methods： 12 patients who were Ken＇s II dentition defect, were repaired for MK--1 precision attachment. Using their own cross--references method, in accordance with the requirements of ceramometal crown preparation, they were worn precision attachment. After 0, 3, 6 months, and relining for 3 months （wear 9 months）, we collected and meas- ured base GCF, reviewed and recorded relevant clinical indexes, determined the absorption of alveolar bone height, and the result of the examination record repair after 3,6,9 months follow--up observation. Results： Precision at- tachment denture with different observation time had different clinical examination index and gingival crevicular fluid content （P^0.05）. Conclusion： MK--1 precision attachment denture after half a year had alveolar bone absorption, denture adaptation decreased, free end teeth＇s stress increasd, a mild periodontal inflammation happened. After heavy lining, denture sink on the abutment improved abutment periodontal tissue recovered, and extended the life of the denture.
