摘 要:目的研究胃食管反流病患者黏膜糜烂与幽门螺杆菌(Hp)感染的相关性,为临床诊治提供参考。方法选择医院2011年2月-2012年11月门诊胃食管反流病患者336例,根据是否合并黏膜糜烂分为两组,黏膜不同程度糜烂的149例患者作为研究组,187例非糜烂性反流病患者为对照组,所有患者均进行Hp检测,对两者的相关性进行研究。结果研究组与对照组Hp感染率分别为34.9%、46.52%,年龄>60岁患者感染率分别为46.98%、28.34%,病程>12个月的感染率分别为44.30%、24.06%,两组差异均有统计学意义(P<0.05);A、B、C、D级患者阳性率分别为41.86%、37.50%、31.25%、23.08%,随着黏膜糜烂程度的加重,Hp阳性率呈下降趋势,C、D级患者Hp阳性明显低于A级,不同分级比较差异有统计学意义(P<0.05)。结论胃食管反流病患者黏膜糜烂与幽门螺杆菌感染呈负相关,糜烂程度越严重,Hp阳性率越低,提示Hp可能对胃食管反流病患者食管糜烂具有保护作用。OBJECTIVE To study the correlation between the mueosal erosions and the Helicobacter pylori (Hp) infection in patients with gastroesophageal reflux so as to guide the clinical diagnosis and treatment. METHODS A total of 336 eases of gastroesophageal reflux patients, who were treated in the hospital from Feb 2011 to Nov 2012, were enrolled in the study and divided into two groups according to the complication of mueosal erosion, 149 cases with different degrees of mucosal erosion were assigned as the study group, and 187 cases without mucosal erosion were set as the control group, then all the patients were tested for the Hp, and the correlation between the two was observed. RESULTS The incidence rate of Hp infections was 34.9% in the study group, 46.52% in the control group; the infection rates of the patients aged more than 60 years old were 46.98% and 28.34%, respectively; the infection rates of the patients with illness course more than 12 months were 44.30% and 24.06%, respectively, the difference between the two groups was significant (P 〈0.05). The positive rates of the patients with grade A, grade B, grade C, and grade D of the mucosal erosion degree were 41.86%, 37.50%%, 31.25%, and 23.08%, respectively, the positive rate of Hp showed a declined trend as the severity of mucosal erosion aggravated; the positive rate of Hp in the patients with grade C or D was significantly lower than that in the patients with grade A, the difference was significant (P〈0.05). CONCLUSION The mueosal erosion is negatively correlated with the Hp infections in the patients with gastroesophageal reflux; the severer the degree of erosion, the lower the positive rate of Hp is, which indicates that the Hp may have a protective effect on the esophageal erosion patients with gastroesophageal reflux.
