摘  要:目的 通过肠系膜根部注射急性胰腺炎相关腹水(PAAF)建立一种损伤因素单一、损伤程度稳定的大鼠回肠损伤模型.方法 将24只SD大鼠随机分3组:正常组(N组);急性坏死性胰腺炎(ANP)组(A组);模型组(M组)予肠系膜根部注射PAAF 0.25 ml.观察24 h后各组大鼠末段回肠组织病理变化,检测血清淀粉酶、白细胞介素(IL)-1β及IL-10浓度,免疫组织化学法检测末段回肠组织中5-羟色胺(5-HT)表达水平,实时荧光定量聚合酶链反应(Real-time PCR)检测末段回肠组织中紧密连接蛋白(TJP-1)的mRNA相对表达水平,评估该模型.结果 A组和M组均出现回肠组织损伤病理改变,A组表现为黏膜下严重充血水肿和出血,甚至黏膜坏死脱落,M组则表现为黏膜损伤、黏膜下炎性细胞浸润.A组血清淀粉酶、IL-1β、IL-10浓度分别为(4623.86±869.22) U/L、(196.77±23.01) ng/L、(536.56±365.46) ng/L,N组为(2251.88±171.13) U/L、(15.68±7.99) ng/L、(228.54±179.02)ng/L,M组为(1170.63土74.20)U/L、(32.17±8.51) ng/L、(189.85±159.22) ng/L,A组显著高于N组和M组(P<0.05),N组和M组之间差异无统计学意义(P>0.05),M组末段回肠组织5-HT表达水平、TJP-1 mRNA相对表达水平均显著低于N组(P<0.05).结论 肠系膜根部注射PAAF致回肠损伤模型可表现为ANP时大鼠的肠道损伤,模型制作过程简单、重复性高,反映了PAAF在ANP肠损伤发病机制中的作用.Objective To establish a rat model of ileum injury with simple injury factor and stable injury degree,by injecting pancreatitis-associated ascitic fluid (PAAF) into the root of mesentery.Methods Twenty-four male SD rats were randomly divided into 3 groups:normal control group (group N),acute necrosis pancreatitis (ANP) group (group A) and model group treated by injecting PAAF 0.25 ml into the root of mesentery (group M).Twenty-four h after establishment,the pathological changes of terminal ileum were observed.The serum concentrations of amylase,interleukin (IL)-1β and IL-10,and the expression of 5-hydroxytryptamine (5-HT) and the relative mRNA expression of tight junction protein 1 (tjp-1) in terminal ileum tissues in each group were measured.The model was evaluated.Results Pathological changes of ileum injury were observed in group A and group M.Severe congestion,edema and hemorrhage in submucosa,even mucosal necrosis and slough were observed in group A.In group M,mucosal injury and submucosal infiltration of inflammatory cells were observed.The serum concentrations of amylase,IL-1β and IL-10 in group A were (4623.86 ± 869.22) U/L,(196.77 ± 23.01) ng/L and (536.56 ± 365.46) ng/L,(2251.88 ±171.13) U/L,(15.68 ±7.99) ng/L and (228.54 ± 179.02) ng/L in group N,and (1170.63 ±74.20) U/L,(32.17 ± 8.51) ng/L and (189.85 ± 159.22) ng/L in group M,respectively.Those in group A were significantly higher than in groups N and M (P < 0.05).There was no statistically significant difference between groups N and M.The expression of 5-HT and the relative mRNA expression of tjp-1 in terminal ileum tissues were significantly lower in group M than in group N (P < 0.05).Conclusion The rat model of ileum injmy by injecting PAAF into the root of mesentery performed the gut injury of ANP rat model.The model was easily producible and repeatable.It could reveal the role of PAAF in the pathogenesis of gut injury in ANP.
