单人操作单气囊小肠镜对小肠疾病的诊断价值

摘　　要：目的评价非麻醉状态下单人操作单气囊小肠镜（SBE）对小肠疾病诊断的临床价值、安全性及耐受性。方法对2010年4月~2012年4月疑有小肠疾病39例患者进行非麻醉状态下单人SBE检查,分析疾病检出率、检查时间、并发症及耐受性。结果 39例患者共进行64例次SBE检查,单纯经口检查7例,单纯经肛检查6例;经口＋经肛检查25例,其中6例完成对接检查;术中小肠镜检查1例。检出病变27例,病变检出率69.2%;经口进镜检查平均时间50.8 min,经肛进镜检查平均时间71.3 min。所有患者均未出现重大并发症,耐受性良好。结论非麻醉状态下单人SBE是一种对小肠疾病诊断价值较高、安全可靠的检查手段。【Objective】To evaluate the clinical efficacy,safety and patients＇ tolerance of single-person operating single balloon enteroscopy（SBE） for diagnosis of small intestinal diseases in which condition patients were under no anesthesia.【Methods】Under no anesthesia,39 patients suspected of small intestinal diseases underwent SBE operated by a single person between April 2010 and April 2012.Positive proportion,procedure time,complications and patients＇ tolerance were included in this retrospective analysis.【Results】A total of 64 procedures were performed in 39 patients.Among all the patients,7 were examined via oral routes and 6 via anal routes,respectively.Among 25 patients in whom procedures were performed both via oral and anal routes,the entire small intestine was explored in 6 cases.SBE was performed in one undergoing surgical operation at the same time.Diseases were detected in 27 of all the patients with the detectable rate of 69.2%.The mean intubation time was 50.8 minutes and 71.3 minutes for the oral and anal routes,respectively.All patients did not suffer from severe complications and presented good tolerance.【Conclusion】Under no anesthesia,SBE is relatively a safe,reliable procedure with high diagnostic value in detecting small intestinal diseases.
