十二指肠Brunner腺瘤病理及临床特点分析(附3例报告)

摘　　要：目的探讨及提高对十二指肠Brunner腺瘤的发病机制、临床病理特点及相关诊治进展的认识。方法总结3例十二指肠Brunner腺瘤的临床及病理资料,并结合相关文献进行分析。结果 患者年龄平均年龄60岁,均在56~64岁,其中有腹胀者1例,伴胆汁反流性胃炎2例,其中1例伴癌胚抗原（CEA）升高。1例行胃肠钡餐造影,考虑十二指肠息肉,行内镜下摘除。另2例均行内镜检查,其中1例考虑十二指肠增生性息肉,另1例考虑增生布氏腺可能,均内镜下取活检。病理诊断均十二指肠Brunner腺瘤。结论十二指肠Brunner腺瘤为一种少见的良性肿瘤,早期临床表现无特异性,与其他息肉难以鉴别,确诊仍有赖于病理形态学特征。【Objective】To investigate and improve the pathogenesis,the clinicopatholgical features,diagnostic modalities and treatment of duodenal Brunner＇s gland adenoma（BGA）.【Methods】3 cases of duodenal BGA were evaluated based on the clinico-pathological features,and their follow-up data were also reviewed.【Results】The mean age of the patients of BGA was 60 years,ranged from 56 to 64 years,including one case associated with abdominal distension,two cases with bile reflux gastritis while one case with carcinoembryonic antigen（CEA） increased.One case was initial diagnosed as duodenal polyps by gastrointestinal barium meal and removed,another initial as duodenal polyps and the other as hyperplasia of Brunner＇s gland by gastric endoscopy while both were performed biopsy.All cases were confirmed duodenal Brunner＇s gland adenoma in pathological diagnosis.【Conclusions】Duodenal Brunner adenoma is a rare benign tumor,the early clinical manifestations of non-specific and difficult to identify with the other polyps,which still relies on pathological morphological features.
