男性少弱精子症不育患者精子DNA碎片检测情况分析

摘　　要：目的研究男性少弱精子症不育患者的精液情况,探讨精子DNA碎片指数(DNA fragmentation index,DFI)预测男性生育力的可行性。方法通过精液常规检查、精子形态学分析以及精子染色质扩散实验等方法检测男性少弱精子症不育患者60例与正常健康生育对照组30例的精液各项指标,并分析DFI与精子密度、前向活动精子、活动率、正常形态率的关系。结果男性不育组的精子密度、前向活动精子、活动率、正常形态率、DFI分别为(41.44±18.73)×106/mL、(30.29±7.20)%、(39.72±10.3)%、(6.88±2.02)%、(30.24±7.59)%;对照组分别为(80.11±42.57)×106/mL、(57.23±8.10)%、(67.24±8.23)%、(20.13±2.19)%、(15.25±4.82)%。两组各指标相比差异均有统计学意义(P<0.05)。DFI与精子密度、前向活动精子、活动率、正常形态率的相关系数分别为-0.472、-0.613、-0.566、-0.816。结论男性少弱精子症不育患者存在精子DNA完整性损伤,DFI对评估男性生育力具有相当重要的临床意义。【Objective】 To explore the feasibility of sperm DNA fragmentation index（DFI） predicting male fertility capacity through investigating the semen of infertile male with oligoasthenospermia.【Methods】 We measured the semen parameters of 60 infertile male with oligoasthenospermia and 30 fertile male with traditional semen examination,morphology analysis and sperm chromatin dispersion test and analyzed the relationship between sperm concentration,forward motile sperm,motility,normal morphology and DFI.【Results】 The sperm concentration,forward motile sperm,motility,normal morphology of infertile male were significantly lower than that of fertile male（41.44 ± 18.73 ×106/mL vs 80.11 ± 42.57 ×106/mL,30.29 ± 7.20% vs 57.23 ± 8.10%,39.72 ± 10.3% vs 67.24 ± 8.23%,6.88 ± 2.02% vs 20.13 ± 2.19%,respectively,P 0.05）,while DFI of infertile male was significantly higher than that of fertile male（30.24 ± 7.59% vs 15.25 ± 4.82%,P 0.05）.The correlation coefficients between DFI and sperm concentration,forward motile sperm,motility,normal morphology were-0.472,-0.613,-0.566,-0.816,respectively.【Conclusion】 Sperm DNA fragmentation has been associated with male infertility.DFI can be a parameter for the prediction of male fertility capacity.
