摘 要:目的:探讨CT平面扫描及增强扫描对妇科盆腔肿瘤样病变的诊断价值。方法:选取2010年3月~2012年3月该院收治的妇科盆腔肿块患者208例,取仰卧位,横断位扫描,自耻骨联合下缘水平向上到病变处上缘。分别进行CT平扫和增强扫描,采用GEAW4.2工作站对扫描图像进行处理,并对良、恶性肿瘤患者CT图像显示的肿瘤边界、盆腔淋巴结肿大情况、肿瘤最大径以及增强幅值进行统计学比较分析。结果:CT检查确诊的良、恶性肿瘤结果与病理检查结果之间差异无统计学意义(P〉0.05);恶性肿瘤与良性肿瘤相比边界清晰的比例明显偏低,差异有统计学意义(P〈0.01);与良性肿瘤相比恶性肿瘤最大径、增强幅值、盆腔淋巴结肿大比例明显偏高,差异有统计学意义(P〈0.01)。结论:CT检查对妇科盆腔肿瘤样病变的鉴别诊断具高准确性,实际工作中要对肿瘤大小、边界、增强幅值以及盆腔淋巴结肿大高度重视,提高诊断准确率。Objective: To explore the values of CT plain scanning and enhancement scanning for diagnosing gynecological pelvic tumor lesions. Methods: A total of 208 patients with gynecological pelvic masses treated in the hospital from March 2010 to March 2012 were selected, then cross CT scanning was conducted, the scanning trend was from the lower edge level of symphysis pubis to the upper edge level of masses. CT plain scanning and enhancement scanning were conducted respectively, GEAW4. 2 workstation was used to deal with the scanning images; the tumor boundary, pelvic lymph node situations, the largest diameters of tumor, and the enhanced amplitudes displayed by CT in patients with benign and malignant tumors were compared and analyzed. Results: There was no significant difference in the diagnostic rates of benign and malignant tumors between CT and pathological examination (P 〉 0. 05) ; the proportion of malignant tumors with clear boundary was statistically significantly lower than that of benign tumors (P 〈 0. 01 ) ; the largest diameter of tumor, enhanced amplitude, the proportion of enlarged pelvic lymph nodes in malignant tumors were statistically significantly higher than those in benign tumors (P 〈 0. 01) . Conclusion: The accuracy of CT scanning for differential diagnosis of gynecological pelvic tumor lesions is high, tumor size, boundary, enhanced amplitude, and pelvic lymph node situation should be paid more attention to improve the accurate rate of diagnosis.
