RUNX3、Survivin在结直肠腺癌、腺瘤、正常黏膜中表达的临床意义

摘　　要：目的通过对人Runt相关转录因子3(RUNX3)、凋亡抑制因子生存素(Survivin)蛋白在结直肠腺癌中表达的研究,探讨RUNX3、Survivin与结直肠腺癌发生发展的关系。方法应用免疫组化(SP)、原位杂交法检测RUNX3、Survivin在30例正常结直肠组织、60例结直肠腺瘤、60例结直肠腺癌组织标本中的表达,分析它们与临床病理特征间的关系。结果 RUNX3、Survivin在结直肠腺癌、正常结直肠组织、结直肠腺瘤伴中低度异型增生和结直肠腺瘤伴重度异型增生中的阳性表达率分别为38.33%、73.33%,86.67%、10.00%,72.97%、13.51%和43.48%、52.17%,两者在正常结直肠组织、结直肠腺瘤伴中低度异型增生组织与结直肠腺癌、结直肠腺瘤伴重度异型增生组织比较,差异有统计学意义(P<0.05)。RUNX3、Survivin mRNA在结直肠癌组织、癌旁正常组织、结直肠腺瘤伴中低度异型增生和结直肠腺瘤伴重度异型增生中的阳性表达率分别为31.67%、63.33%,76.67%、6.67%,56.76%、10.81%和39.13%、43.48%,两者在正常结直肠组织、结直肠腺瘤伴中低度异型增生组织与结直肠腺癌、结直肠腺瘤伴重度异型增生组织比较,差异有统计学意义(P<0.05)。RUNX3和Survivin在结直肠腺癌组织中的表达呈负相关(r=-0.455,P<0.01)。RUNX3和Survivin的表达与结直肠腺癌的TNM分期、浸润深度、淋巴结转移、肝转移具有相关性(P<0.05)。结论 RUNX3、Survivin在结直肠腺癌中的不同程度的表达与结直肠黏膜癌变的发生、发展有关。Objective To study the expression of RUNX3 and Survivin in human colorectal adenocarcinoma tissues and explore their correlation with the initiation and progression of colorectal cancer. Methods The expression of RUNX3 and Survivin in 30 specimens of normal colorectal tissues,60 specimens of colorectal adenoma tissues,and 60 colorectal adenocareinoma tissues were detected using immunohistochemistry technique and in situ hybridization.The correlation between their expression and the elinieopathologie features was analyzed. Results The positive rates of RUNX3 and Survivin in colorectal adenocarcinoma tissues were 38.33% and 73.33% respectively, while in 30 normal colorectal tissues the expression was 86.67% and 10.00%, and in 60 colorectal adenoma tissues with 37 mild dysplasia and 23 severe dysplasia, the expression was 72. 97%, 13.51% and 43.48%, 52. 17%. The difference was significant statistically between colorectal adenocarcinoma, colorectal adenoma with severe dysplasia and normal colorectal tissues ,colorectal adenoma with mild dysplasia（ P 〈 0. 05 ）. The positive rates of RUNX3 mRNA and Survivin mRNA in colorectal adenocarcinoma tissues were 31.67% and 63.33%, while in 30 normal colorectal tissues the expression was 76. 67% ,6.67%, and in 60 colorectal adenoma tissues with 37 mild dysplasia and 23 severe dysplasia, the expression was 56.76% ,10. 81% and 39. 13% ,43.48%. Thedifference was significant statistically between colorectal adenocarcinoma, colorectaLadenoma with severe dysplasia and normal colorectal tissues, colorectal adenoma with mild dysplasia（ P 〈 0. 05 ）. Expression of RUNX3 was negatively related with expression of Survivin in colorectal cancer tissues （ r = - 0. 455, P 〈 0. 01 ）. Expression levels of RUNX3 and Survivin were correlated with the TNM staging, liver metastasis, the depth of tumor invasion, and lymph node metastasis （ P 〈 0.05 ）.
