儿童血管瘤和血管畸形初诊病例临床特点分析

摘　　要：目的了解血管瘤和血管畸形的临床特点,以期为该类疾病早期预防及治疗提供依据。方法采取调查问卷方式收集近3年来我院门诊确诊的1 845例患儿,分析该类疾病的临床特点。结果①总体资料特点:男女比例1∶1.25;初诊年龄82.3%的患儿<1个月;部位以头、面部为主;皮损面积以<5 cm2最多;并发症以溃疡、出血最多;治疗上采用随访最多。②疾病分布:草莓状血管瘤1 225例(66.4%),其后依次为鲜红斑痣344例(18.65%)、血管痣154例(8.35%)、海绵状血管瘤53例(2.87%)、毛细血管扩张45(2.44%)、化脓性肉芽肿19例(1.03%)、血管角皮瘤3例(0.16%)、淋巴管畸形和混合畸形2例(0.11%)。结论在该类疾病中,草莓状血管瘤所占比例最大。血管瘤女性发病率高于男性,皮损主要分布于面部,治疗上多采取随访。Objective To summarize and analyze the clinical characteristics and treatment of 1 845 cases of childhood hemangioma and vascular malformation, and to provide information for early prevention and proper treatment. Methods Questionnaires were used to collect the clinical data of 1 845 cases of children patients at the Depart- ment of Dermatology in Anhui provincial Children＇ s hospital in last 3 years, analyzed the clinical characteristics of the disease. Results The male to female ratio was 1 ： 1.25. The onset age of most cases （82.3 % ） was within one month. The lesions were mainly located on the head and face. The size of lesions in most patients was within 5 em2. Ulcers, bleeding were the main complications. Among 1 845 cases, strawberry hemangiomas was mostly seen, accounting for 66.4% （1 225 cases） ,followed by nevus flammeus （18.65% ,344 cases） ,then spider telangiectasia （8.35%, 154 cases）, capillary Abnormality （2.44% ,45cases）, pyogenic granuloma （ 1.03%, 19 cases）, angiokeratoma corporis diffusum （0. 16% ,3 cases）, lymphatic malformations（0. 11% ,2 cases）. Conclusion Srawberry hemangiomas is the most common hemangioma and vascular malformation disease in children. Their incidences is higher in female than that in male. Hemangioma and vascular malformation has an earlier onset age. Skin injuries are mostly seen in face. Most pediatric patients experience good prognosis after being treated with watching and waiting.
