1218例脑血管病患者主动脉弓血管形态改变及其临床意义

摘　　要：目的通过主动脉弓造影观察主动脉弓的病理形态改变,为介入治疗提供参考依据。方法对1218例脑血管病患者,常规行主动脉弓造影,按照Casserly分型将主动脉弓分为I型、II型、III型,分析总结主动脉弓病理解剖形态改变,比较各型主动脉弓与年龄、性别、高血压、糖尿病、高血脂相关性。结果 (1)主动脉弓呈I型545例(44.7%),呈II型382例(31.4%),呈III型291例(23.9%);(2)年龄和高血压病史与主动脉弓形态有相关性,高龄和高血压病史的患者,主动脉弓呈III型病理改变明显,性别、高血脂、糖尿病病史与主动脉弓形态无相关性。结论主动脉弓存在着多种血管病理形态改变,注意高龄及高血压患者主动脉弓形态改变,本研究对提高脑血管介入手术成功率和诊疗效果有重要参考价值。Objective To observe the pathological morphological changes of the aortic arch by aorta angiography, provided the reference basis for interventional therapy. Methods 1218 cases of cerebrovascular disease with the conven- tional line aorta angiography were divided into type I, II, III by pathology classification of Casserly. Analysed and summaried the aortic arch anatomic morphology change, compared the correlation among aortic arch with age, sex, hypertension, diabetes and hyperlipidemia. Results Aortic arch in type I had 545 cases （44.7%） , type 11 had 382 cases （31.4%） and type m had 291 cases （23. 90/o ）;Both age and medical history of high blood pressure were correlated with the morphology of aortic arch. The patients with the history of high blood pressure or the elder had type III pathological changes obviously in aortic arch. But gender, history of high cholesterol, and diabetes had no correlation with the morphology of aortic arch. Con- clusion There were many kinds of vascular pathological morphological changes in aortic arch. We should pay more atten- tion to the elder and the hypertension patients with the change of aortic arch morphology. The study had the important refer- ence value to improve successful rate and diagnosis effect of cerebrovascular intervention operation.
