卵巢癌组织激肽释放酶7表达的临床意义

摘　　要：目的观察激肽释放酶（KLK）7基因表达与卵巢上皮性癌的关系，探讨卵巢上皮性癌的发生机制及相关基因的功能，寻找卵巢上皮性癌早期诊断、预后检测的新指标。方法采用逆转录聚合酶链反应（RT—PCR）检测40例卵巢上皮性癌、44例卵巢良性肿瘤和40例正常卵巢组织中KLK7的表达，分析KLK7基因表达与卵巢上皮性癌临床病理特征的关系。结果KLK7在正常卵巢组织中未见表达，KLK7在卵巢上皮性癌的阳性表达率为77．50％（31／40），显著高于良性肿瘤的22．73％（10／44），两者比较差异有统计学意义（P〈0．01）。且合并淋巴结转移标本的阳性表达率为91．67％（22／24），高于无合并淋巴结转移标本的56．25％（9／16），两者比较差异有统计学意义（P〈0．05）。但不同临床分期、不同组织类型卵巢癌组织中表达量差异均无统计学意义。结论KLK7在正常卵巢组织中无表达，在卵巢上皮性癌组织中高表达，尤其在合并淋巴结转移的卵巢上皮性癌中显著高表达。提示KLK7可能是一种新的反应卵巢上皮性癌恶性程度和预后的肿瘤标记物。Objective To observe the relationship between the expression of kallikrein （KLKT） and human ovarian epithelial cancer, investiagate the pathogenesis of ovarian epithelial cancer, the relat-ed genes function, and find out the new index for early diagnosis and prognosis detection of ovarian epi-thelial cancer. Methods Reverse transcription-polymerase chain reaction （RT-PCR） method and agar-ose gel electrophoresis were employed to detecte the expression of kallikrein 7 （ KLK7 ） in human normal o-varian tissues, benign tumor tissues and epithelial ovarian cancers. Results The positive expression rate of KLK7 in normal ovarian tissues was zero. The positive expression rate of KLK7 in epithelial ovarian cancers was 77. 50% （31/40）, which was higher than that in benign tumor（22. 73% ,10/44）, there was significant difference （P 〈 0. 01 ）. And the positive expression rate of KLK7 in epithelial ovarian cancer with node metastasis was 91.67% （22/24）, which was higher than that in epithelial ovarian cancer without node metastasis（56. 25% ,9/16）, there was significant difference （P 〈 0. 05）. But there was no significant difference in KLK7 expression of ovarian tissues between different clinical stages and histological grades. Conclusions The expression rate of KLK7 in normal ovarian tissues is zero. The expression of KLK7 in o-varian epithelial cancer tissues is higher, especially in the ovarian cancer tissues with lymph node metasta-sis. So KLK7 maybe a new epithelial ovarian tumor markers that can predict the prognosis of the patients.
