颅内基底动脉瘤介入治疗的临床观察

摘　　要：目的探讨经血管内治疗颅内基底动脉瘤的安全性和有效性。方法56例颅内基底动脉瘤患者中基底动脉顶端动脉瘤10例，基底动脉干动脉瘤46例；夹层动脉瘤20例，囊状动脉瘤19例，梭形动脉瘤17例。未破裂动脉瘤20例，发生过破裂出血36例。在全身麻醉下进行血管内治疗，观察其安全性和治疗效果。结果56例患者技术成功率为100％，术后均未发生再出血，无新增神经功能缺失，无死亡病例，随访2～72个月，原有症状均获明显改善。夹层动脉瘤20例中单纯支架置入8例，支架辅助弹簧圈栓塞12例，动脉瘤未见显影，支架位置理想，支架内血流通畅；囊状动脉瘤19例中完全栓塞17例，次全栓塞1例，部分栓塞1例；梭形动脉瘤17例中单纯支架置入13例，双支架置入4例，瘤体未见显影，支架位置理想，支架内血流通畅。结论血管内介入治疗为基底动脉瘤的治疗提供了微创和较为安全、有效的治疗方法。Objective To investigate the safety and efficacy of endovascular treatment of in-tracranial basilar aneurysm. Methods Among the 56 cases of patients with basilar aneurysms, top of basilar aneurysm in 10 cases, basilar aneurysm in 46 cases; The dissecting aneurysm in 20 cases, cystic aneurysm in 19 cases, fusiform aneurysm in 17 cases. Un-ruptured aneurysm in 20 cases, ruptured aneu-rysm in 36 cases. Patients all underwent general anesthesia for endovascular treatment, and then the se-curity and effects were observed. Results Success rate of technology in 56 cases were 100% , none of the postoperative re-bleeding, no additional loss of nerve function, no death case, followed-up for 2 - 72 months, the original symptoms were significantly improved. Dissecting aneurysm in 20 cases, simple bracket reset in 8 cases, stent-assisted coil embolization in 12 cases, aneurysm was not developing, stent location was ideal, blood flow in the stent smoothly ; 19 cases of saccular aneurysm, the complete embol-ism in 17 cases, subtotal embolism in 1 case, part embolism in 1 case; fusiform aneurysm in 17 cases, stent alone in 13 cases, double stenting in 4 eases, aneurysm was not developing, stent location was ide-al, blood flow in the stent smoothly. Conclusions Endovascular interventional treatment provide mini- mally invasion, safety and effective method for basilar artery aneurysm.
