摘 要:目的探讨肝脏局灶性结节增生(focal nodular hyperplasia,FNH)在应用肝胆特异性磁共振对比剂钆塞酸(普美显)Gd-EOB-DTPA行MR三期增强扫描及延迟20min肝胆期的特异性MR表现,从而达到提高对FNH诊断准确率。方法分析经手术或穿刺活检病理证实的行MR检查患者15例。结果 15例患者MR平扫共发现18个病灶,其中14个病灶均为T1WI稍低或等信号,T2WI等或稍高信号,2个病灶T1、T2呈等信号,2个病灶T1、T2呈稍高信号;8个病灶内含典型的中心疤痕,在T1WI为低信号、T2WI上为高信号;增强后:动脉期病灶见明显强化,门脉期及平衡期病灶呈等或稍高信号,其中中心瘢痕在动脉期及门脉期呈低信号,平衡期有轻度强化。延迟20min的肝胆期病灶呈等信号或稍高信号,中心瘢痕为低信号,3个病灶内可见高信号胆管影;4个病灶有假包膜,在T2上为高信号、延迟有强化。结论平扫加应用肝胆特异性磁共振对比剂Gd-EOB-DTPA的三期动态增强MR及延迟20min肝胆期能较好反映的FNH影像学特点,尤其是应用Gd-EOB-DTPA后肝胆期特异性MR表现可以提供更多的诊断相关信息,显著提高其与其他肝脏肿瘤的鉴别诊断能力并提高诊断信心。Objective To investigate the hepatic focal nodular hyperplasia (focal nodular hyperplasia, FNH) acid Gd-EOB-DTPA MR three phase enhanced scanning and plug in the application hepatobiliary-specific magnetic resonance contrast agent gadolinium delayed by 20 minutes of the specificity of MR imaging of the hepatobiliary phase, so as to improve the accuracy of the diagnosis of FNH. Medthods Analysis MR examinations of 15 patients confirmed by surgery or biopsy pathology. Results 15 patients of MR plain scan found a total of 18 lesions, 14 lesions were the T1WI slightly lower or equal signal, T2WI, or slightly higher signal, two lesions in T1, T2 was signal, two lesions in T1 and T2 was slightly higher signal; eight lesions containing typical central scar, low signal in T1WI, T2WI high signal; enhanced: arterial phase, see significantly enhanced portal venous phase and equilibrium phase lesions showed equal or slightly higher signal, central scar in the arterial phase and portal venous phase showed low signal, the balance of mild enhancement. Hepatobiliary of the lesions were delayed by 20 minutes of signal or slightly hyperintense central scar and low signal, high signal bile duct lesions seen in shadow; the four lesions fake envelope, high signal on T2 delay strengthen. Conclusion plain and hepatobiliary-specific magnetic resonance contrast agent Gd-EOB-DTPA has been the three dynamic contrast-enhanced MR and 20 minutes delayed hepatobiliary can reflect FNH imaging features, especially the application of Gd-EOB-DTPA hepatobiliary period the specificity of MR imaging can provide more diagnostic information, significantly improve the ability of the differential diagnosis with other liver tumors and improve diagnostic confidence.
