摘 要:目的:探讨超声弹性成像联合常规超声在乳腺良恶性病变鉴别诊断中的价值。方法:回顾分析2009年1月—2010年12月经手术病理证实的270例共330个乳腺良恶性病变的常规超声及超声弹性成像检查结果,分别作出常规超声、超声弹性成像及联合应用的超声诊断,结果与手术病理对照。结果:常规超声、超声弹性成像及联合应用诊断乳腺病灶良恶性的敏感性分别为86.3%(101/117)、70.9%(83/117)及94.0%(110/117);特异性分别为92.0%(196/213)、89.2%(190/213)及94.8%(202/213);准确性分别为90.0%(297/330)、82.7%(273/330)及94.5%(312/330)。常规超声和超声弹性成像联合应用与单独应用相比,敏感性、特异性及准确性差异均有统计学意义(P<0.05)。结论:联合应用超声弹性成像和常规超声有助于提高乳腺病灶良恶性的鉴别诊断能力。Objective:To explore the value of combined application of ultrasonic elastography and conventional ultrasonography in the differential diagnosis of breast lesions.Methods:A total of 330 breast lesions in270 patients proved by surgical-pathology from Jan 2009 to Dec 2010 were examined with conventional ultrasonography and ultrasonic elastography.The sonograms and elastograms were analyzed.The results were compared with pathology.Results:The sensitivities of conventional ultrasonography,ultrasonic elastography and combined application of the two techniques in the differential diagnosis of malignant breast lesions were 86.3%(101/117),70.9%(83/117)and94.0%(110/117),respectively;the corresponding specificities were 92.0%(196/213),89.2%(190/213)and 94.8%(202/213),respectively;the corresponding accuracies were 90.0%(297/330),82.7%(273/330)and 94.5%(312/330),respectively.Combined application of the two techniques could significantly improve the sensitivity,specificity and accuracy,compared with conventional ultrasonography and ultrasonic elastography.Conclusions:Combined application of conventional ultrasonography and ultrasonic elastography can improve differential diagnostic ability in breast lesions.
