摘　　要：目的探讨基质金属蛋白酶-7(MMP-7)和碱性成纤维细胞生长因子(FGF)在子宫内膜样腺癌中的表达及其临床意义。方法采用免疫组织化学法检测原发性子宫内膜样腺癌、非典型增生子宫内膜和正常子宫内膜组织中MMP-7和bFGF蛋白的表达水平。结果 1.不典型增生子宫内膜组、子宫内膜样腺癌组的MMP-7和bFGF阳性率均明显高于正常组(P均<0.01),不典型增生组与子宫内膜样腺癌组比较,无明显差异。2.MMP-7和bFGF的阳性率在子宫内膜样腺癌组手术-病理分期中,Ⅱ期和Ⅲ-Ⅳ期合并后与Ⅰ期比较,差异均有统计学意义(P均<0.05);低-中分化腺癌(G3-G2)与高分化腺癌(G1)相比,均有明显差异(P<0.05);浸润程度>1/2组和浸润程度≤1/2组合并后与无肌层浸润组比较,有显著性差异(P均<0.01)。淋巴结转移组与无淋巴结转移组比较,亦具有统计学差异(P<0.01)。3.子宫内膜样腺癌中MMP-7和bFGF的表达呈正相关(r=0.367,P=0.010)。结论 MMP-7与bFGF蛋白的联合检测可能作为判断子宫内膜样腺癌发生发展和评价预后的有效参考指标之一。Objective To investigate the expressive clinical significance of matrix metalloproteinase-7（MMP-7） and basic fibroblast growth factor（bFGF） in endometrioid carcinoma.Methods The expression of MMP-7 and bFGF in endometrioid carcinoma,atypical hyperplasia endometrium and normal endometrium were detected by immunohistochemistry.Results 1.The positive rate of MMP-7 and bFGF was higher both in endometrial atypical hyperplasia and endometrioid carcinoma group than that in normal endometrium（P ＆lt; 0.01）,and was no difference beween endometrioid carcinoma and endometrial atypical hyperplasia group（P ＆gt; 0.05）.2.The protein expression of MMP-7 and bFGF,in both staging Ⅱand Ⅲ-Ⅳendometrioid carcinoma group was significantly different from the stagingⅠgroup（P ＆lt;0.05）;In G2-G3 group of endometrioid carcinoma was also significantly different from G1 group（P ＆lt; 0.05）;In both invasive depth ≤1 /2 and ＆gt; 1 / 2 group was obviously different from the group without myometrial invasion（P ＆lt; 0.01）;In group with Lymph node metastasis was also significantly different from that in group without Lymph node metastasis（P ＆lt; 0.01）.3.The expression of MMP-7 was positively correlated to bFGF in endometrioid carcinoma（r = 0.367,P = 0.010）.Conclusion Combined detection of MMP-7 and bFGF may be a effective parameter in evaluate occurrence,progression and prognosis of endometrioid carcinoma,and may also have important clinical significance in early prediction of endometrioid carcinoma.
