CASP7基因多态性与食管癌淋巴结转移易感性的关联研究

摘　　要：目的研究细胞凋亡通路相关基因caspase7（CASP7）单核苷酸多态性与中国汉族人群食管癌患者淋巴结转移风险的关系。方法采用SNPscanTM高通量基因分型技术分析380例食管癌患者有淋巴结转移组（n＝85）和无淋巴结转移组（n＝270） CASP7 rs3127075 G＞C和CASP7 rs7907519 C＞A这两个多态性位点的野生基因型、杂合基因型及突变纯合基因型频率，计算各种基因型的食管癌淋巴结转移风险及其95％可信区间。结果 Logistic回归分析中，调整性别、年龄、吸烟及饮酒因素后，以 CASP7 rs7907519 CC基因型作为参照， CASP7 rs7907519 AA基因型能显著增加食管癌淋巴结转移风险（调整 OR＝3.08，95％ CI＝1.03-9.19）。在隐性模型中，相对于CC＋CA基因型，CASP7 rs7907519 AA基因型与食管癌淋巴结转移风险显著相关（调整 OR＝3.23，95％ CI＝1.11～9.43）。对CASP7 rs3127075 G＞C多态性位点进行相同分析，没有发现其在食管癌淋巴结转移组和无淋巴结转移组间差异有统计学意义。结论 CASP7 rs7907519 AA基因型能显著增加食管癌淋巴结转移风险，提示CASP7 rs7907519 C＞A可能作为预测食管癌患者淋巴结转移情况以及判断患者预后的重要分子生物学指标。Objective To investigate the association between CASP7 rs3127075 G 〉 C and CASP7 rs7907519 C〉A polymorphisms and the risk of esophageal cancer lymphatic metastasis in a Chinese Han popula-tion .Methods A custom-by-design 2 × 48-Plex SNPscanTM Kit was used to analyze the genotypes of target poly-morphic sites of 85 lymphatic metastasis positive esophageal cancer patients and 270 lymphatic metastasis negative patients .Results Logistic regression analyses adjusted for age ,sex ,smoking status and alcohol consumption re-vealed that CASP7 rs7907519 AA genotype contributed to a significant increased risk of lymphatic metastasis com-pared with its CC genotype （adjusted OR =3.08 ,95% CI =1.03-9.19） .We also found a notable relationship be-tween CASP7 rs7907519 AA genotype and the susceptibility to lymphatic metastasis in esophageal cancer in a re-cessive model （adjusted OR =3.23 ,95% CI =1.11-9.43） .There was no significant difference in genotype fre-quency of CASP7 rs3127075 G〉C between lymphatic metastasis positive esophageal cancer patients and lymphatic metastasis negative patients .Conclusion CASP7 rs7907519 AA genotype may be a risk factor of lymphatic me-tastasis .We can make a plausible putative result that CASP7 rs7907519 C〉A polymorphism can serve as an im-portant indicator to predict lymphatic metastasis and evaluate patients＇prognosis .
