摘  要:目的 通过研究弥散张量成像反映的脑膜瘤周围神经纤维结构变化与功能受损的关系,评价DTI在脑膜瘤中的临床应用价值.方法 对23例脑膜瘤患者的肌力及语言功能受损情况与基于DTI的皮质脊髓束、上纵束的结构改变进行相关性分析,比较功能受损组与无损伤组患者相应纤维束的受压及FA值降低数.结果 2组患者的纤维束受压数有显著性差异,FA值降低数则无显著性差异.结论 脑膜瘤对周围白质纤维的压迫与其功能损伤有一定的关系,受压反映神经功能损伤的敏感性较高,脑膜瘤周围白质FA值的降低在反映神经结构破坏中的作用不大.Objective To estimate the clinical application value of diffusion tensor imaging (DTI) in meningioma by analyzing the relationship between structural changes of peritumoral neural fibers derived from dTI and functional damage. Methods Functional damage of muscle strength , language and struclure change of the corticospinal tract and superior longitudinal fasciculus were analyzed in 23 patients with meningioma. Fiber deviation and decrease of fractional anisotropy (FA) were compared between patients with and without functional damage. Results There wasa significant difference for the numberof fiber deviation between two group, and no significant difference of FA decrease between two group. Conclusion The fiber deviation contributes to the functional damage and is sensitive to predict the function damage. Decrease of FA of white matter around meningioma has little help in revealing lhe neural structure destruction.
