治疗单纯疱疹病毒性角膜炎52例临床体会

摘　　要：目的分析抗病毒药及抗病毒药联合干扰素治疗单纯疱疹病毒性角膜炎的临床效果。方法对于2005年7月至2009年7月在大连开发区医院眼科收治的单纯疱疹病毒性角膜炎患者52例58只眼，分为治疗组和对照组，治疗组34只眼，对照组24只眼。对照组用1g／L阿昔洛韦点眼，日5-7次。治疗组除同对照组治疗外，给予干扰素10万U球结膜下注射，每日或隔日1次，每次1ml，共5～7次；同时，成人肌肉注射干扰素100万U，儿童肌肉注射100万U／m^2，均为隔日一次，共5～7次。结果治疗组和对照组治愈率分别为73．53％和37．50％，总有效率为94．12％和54．17％，两组治愈率及有效率比较差异均有统计学意义（P〈0．05）。治疗组视力提高32只眼，对照组视力提高13只眼。结论全身及结膜下注射干扰素及联合抗病毒药物治疗单纯疱疹病毒性角膜炎治愈率高、副反应少，临床效果良好．Objective To evaluate the clinical efficacy of antiviral drugs alone and interferon combined with antiviral drugs in the treatment of herpes simplex viral keratitis. Methods A total of 52 patients （58 eyes） were collected, and randomly divided into treatment group （34 eyes） and control group （24 eyes）. Acyclovir eye drop （lg.L-1） was administrated in the control group for 5 to 7 times daily. Besides the same treatment as control group, 100 thousands units interferon were administrated by subconjunctival injection for 5 to each two days later, 1 ml one time. Intramuscular 7 times totally, once everyday for three days and injection of 1 million unites interferon was admin-istrated at the same time in adult and 1 million U.m-2 in children, 5 to 7 times totally and once each two days. Results The curative rate was 73.53% in treatment group, and 37.5% in control group, The total effective rates was 94.12% in treatment group, and 54.17% in the control group. There were significant differences in curative rates and total effective rates between the two groups （both P 〈0.05）. Visual acuity of 32 eyes increased in treatment group, and 13 eyes in control group. Conclusions Intramuscular and subconjunctival injection of interferon combined with antiviral drugs is with high curative rate, seldom side effects and good clinical results for herpes simplex viral keratitis.
