摘 要:目的 分析年龄相关性白内障患者的角膜内皮细胞(corneal endothelial cell,CEC)特征,探讨定量分析眼球衰老的可能性。方法选取我院2011年8月至12月手术治疗的年龄相关性白内障患者199例(211眼),根据晶状体核硬度分为Ⅱ级核组、Ⅲ级核组和Ⅳ级核组,再根据年龄大小分为〈65岁、65~80岁和〉80岁3个亚组。利用角膜内皮显微镜测量各组中央角膜厚度(central corneal thickness,CCT)、CEC细胞密度(cell density,CD)、六角形细胞比例(percentage of hexagonal cells.HEX)和变异系数(coemeient of variation,CV)。结果各级别晶状体核各年龄亚组间CCT和CD差异均无统计学意义(均为P〉0.05)。Ⅲ级核组内〈65岁亚组HEX为(69.18±19.16)%,较65~80岁亚组的(54.18±17.33)%高,差异有统计学意义(P〈0.05)。Ⅱ级核组、Ⅳ级核组内各年龄亚组HEX差异均无统计学意义(均为P〉0.05)。Ⅳ级核组CV均较Ⅱ级核组和Ⅲ级核组大,差异均有统计学意义(均为P〈0.01),Ⅱ级核组和Ⅲ级核组间CV差异无统计学意义(P〉0.05);Ⅱ级核组内〈65岁亚组CV为(23.55±2.61)%,较〉80岁亚组的(25.76±2.47)%小,Ⅲ级核组内〈65岁亚组CV为(23.09±2.91)%,较其他亚组小,差异均有统计学意义(均为P〈0.05);IV级核组内各亚组CV差异均无统计学意义(均为P〉0.05)。结论不同晶状体核级别、不同年龄的年龄相关性白内障患者CECCD基本相同,但存在形态差异,对CEC形态学的检查可定量反映年龄相关性白内障患者眼王j己衰老程唐。Objective To analyze the corneal endothelial cells in patients with age-related cataract and discuss the possibility of quantitative analysis of eye aging. Methods One hundred and ninty-nine patients with age-related cataract (211 eyes) who received operation treatment in our hospital from August to December in 2011, based on the lens nucleus hardness, were divided into Ⅱ, Ⅲ and IV nuclear-grade groups,and further divided into three subgroups as 〈65 years old,65 -80 years old and 〉 80 years old according to age. The thickness of central corneal, the density of CEC cells ,the percentage of hexagonal cells and the coefficient of variation of all groups were measured by the corneal endothelial microscope. Results The CCT and CD be- tween the levels of lens nucleus, and between the subgroups were not statistically signif- icant(all P〉 0.05). The m class nuclear group 〈65 years old subgroup HEX (69.18 ±19.16)% was higher,compared with the 65 - 80 years old groups (54. 18 ± 17.33)% , the difference was statistically significant(P 〈 0.05 ). The HEX difference in Ⅱ and m class nuclear groups had no statistical significance (P 〉 0.05 ). Compared with Ⅱ and m class nuclear group ,the CV in IV class nuclear group was bigger with (23.55 ±2.61 )% difference, and was smaller than 〉 80 years old sub group ( 25.76 ± 2.47 ) % , CV in the m class nuclear group 〈65 subgroup was (23.09 ±2.91 )% ,compared with other sub- group, this group was smaller, and the differences were statistically significant ( all P 〈 0.05 ) ;The difference of CV in sub-groups of IV class nuclear group had no statistical significance (P 〉 0.05 ). Coneluslon CEC and CD is basically the same in differentlens nuclear level and different age of patients with age-related cataract, but there are morphological differences. The in- spection of CEC morphology can quantitatively reflect the degree of eye aging in patients with age-related cataract.
