摘 要:目的:改变目前医院以科室收支差为基础的传统绩效管理方式,探索建立同时体现医疗工作量和医疗质量的新型绩效管理体系。方法:以新医改政策为导向,基于理论与实践研究初步构建以工作数量考核和工作质量考核为依据的"二元均衡"绩效管理体系,通过专家咨询方法确立指标体系并进行医院实践应用,同时定期采用TOPSIS、DEA方法对医疗质量和运营效率进行跟踪评价,用以实现对绩效方案的持续改进。结果:成功构建"二元均衡"绩效管理体系,在多家医院实践应用并取得效果良好。结论:"二元均衡"绩效管理体系以患者为中心,实现了医疗工作量和医疗质量的二元均衡绩效考核和支付,体现了"多劳多得,优质优酬"的分配理念和原则。Objective: To change the current traditional performance-related management mode based on financial surplus in hospitals, explore a new performance management system of considering both the quantity of work and quality of medical care. Methods:Oriented by the new health reform policy, the indicator set of the “Dual Balance” performance management system in health services was established by the literature reviews and Delphi method and has been practiced in several hospitals. Meanwhile TOPSIS and DEA methods were employed to track the medical quality and operational efficiency for the continuous improvement of performance evaluation. Results: “Dual balanced” performance management system was successfully constructed and the practical applications in hospitals show that it has achieved good effectives. Conclusion: “Dual balanced” performance management system of “patient-centered” achieves the binary balanced performance appraisal and payment in the work quantity and quality, which embodies the concepts and principles of“more pay for more work, better pay for better quality” .
