摘 要:目的探讨人类斯钙素-1(hSTC-1)基因在肾细胞癌患者静脉血中的表达及其与肿瘤恶性程度的关系,为肾细胞癌的诊断提供一种新的肿瘤标志物。方法采用反转录聚合酶链反应检测25例肾细胞癌患者(肿瘤组)和25名非肿瘤人群(包括5名妊娠期妇女,对照组)的静脉血hSTC-1mRNA,并将肾细胞癌患者静脉血hSTC-1的表达结果与临床病理资料进行比较。同时采用受试者工作特征曲线(ROC曲线)进行灵敏度和特异度的综合评价。结果肿瘤组的静脉血hSTC-1基因表达总阳性率为56%(14/25),不同性别、年龄、病理分型间的hSTC-1基因表达阳性率的差异均无统计学意义(P值均〉0.05),肾细胞癌Ⅲ期和Ⅳ期患者的表达阳性率显著高于Ⅰ期和Ⅱ期患者(P=0.007),伴有远处淋巴结转移者的表达阳性率显著高于无远处淋巴结转移者(P=0.042)。对照组中无1例患者的静脉血hSTC-1基因表达阳性。ROC曲线下面积为0.784,95%可信区间为0.646~0.914(P〈0.01)。结论静脉血hSTC-1基因对肾细胞癌可能有较高的诊断价值,并有望成为诊断肾细胞癌和判断其恶性程度的新的肿瘤标志物。Objective To explore the expression of human stanniocalcin-1(hSTC-1)in venous blood of patients with renal cell carcinoma and its relationship with tumor grade,so as to provide a novel tumor marker for the diagnosis of renal cell carcinoma.Methods The expression of hSTC-1mRNA in venous blood of 25patients with renal cell carcinoma and 25people without tumor(including 5pregnant women)was determined by reverse transcription polymerase chain reaction(RT-PCR).The results were compared with clinical pathological data.Comprehensive evaluation of sensitivity and specificity was conducted by receiver operating characteristic(ROC) curve.Results Among the 25patients with renal cell carcinoma,14(56%)showed positive expression of hSTC-1.There was no significant difference in hSTC-1 mRNA expression among different genders,ages or pathological types(all P0.05),while there was statistical difference in hSTC-1mRNA expression between stageⅠ-Ⅱ and stageⅢ-Ⅳ of renal cell carcinoma(P=0.007),and between the patients with and without distant lymph node metastasis(P=0.042).hSTC-1was not found in the venousl blood of people without tumor.ROC area under curve was 0.784,and 95% confidence interval was 0.646-0.914(P0.01).Conclusion The mRNA level of hSTC-1in venous blood has a high diagnostic value for renal cell carcinoma.It may be a novel tumor marker for diagnosis and staging of renal cell carcinoma.
