摘 要:目的探讨微血管密度(MVD)在肾细胞癌中的表达及其与肿瘤临床病理特征的关系。方法采用免疫组织化学法检测53例。肾癌组织、46例癌旁肾脏组织和34例正常肾脏组织中CD34的表达,并计算MVD,分析肿瘤临床病理特征与MVD的关系。结果。肾癌组织中微血管丰富,可见大量小的血管聚集所形成紧密的聚合体,癌旁和正常肾脏组织的血管密度稀疏,但大血管较多。肾癌组织的平均MVD为59.54±18.22,显著高于正常肾脏组织的23.35土7.55和癌旁肾脏组织的48.94±14.49(P值均〈O.01);癌旁肾脏组织的MVD亦显著高于正常肾脏组织(P〈O.01)。Fuhrman分级I级和Ⅱ级患者的MVD显著高于Ⅲ级和Ⅳ级患者(P〈O.01),肿瘤直径〉7crri患者的MVD显著高于肿瘤直径〉4~7cm和≤4cm的患者(P值分别〈0.05、0.01),美国癌症联合委员会(AJCC)分期Ⅲ期和Ⅳ期患者的MVD显著高于I期和Ⅱ期患者(P〈O.01),男性与女性患者间MVD的差异无统计学意义(P〉0.05)。结论肾癌组织的MVD高于癌旁。肾脏组织和正常肾脏组织,且MVD与肿瘤的分级、分期和大小有关。Objective To investigate the expression of microvessel density (MVD) in renal cell carcinoma (RCC) and its relationship with tumor clinicopathologic features. Methods The expression of CD34 was detected in 53 RCC tissues, 46 adjacent tumor tissues and 34 normal renal tissues by immunohistochemistry. MVD was counted and the relationship between MVD and clinicopathologic features of tumor was analyzed. Results Abundant microvessels were detected in RCC, and a lot of small vessels gathered and formed syncytiums. But there was scant vascularity and some large vessels in adjacent tumor tissue and normal renal tissue. The MVD level in RCC was 59.54 -- 18.22, which was significantly higher than adjacent tumor tissue (48.94±14.49, P〈0.01) and normal renal tissue (23.35±7.55, P〈0.01). And MVD in adjacent tumor tissue was significantly higher than that in normal renaltissue (P〈0.01). MVD in Fuhrman I and Ⅱ patients was significantly higher than that in FuhrmanⅢ and Ⅳ patients (P〈0.01). Patients with tumor 〉7 cm had higher MVD compared with patients with tumor≤4 cm (P〈0.01) and tumor between 4 and 7 cm (P〈0.05). MVD in AJCC Ⅲ and Ⅳ patients was significantly higher than AJCC I and Ⅱ patients (P〈0. 01). There was no statistical difference in MVD between females and males (P〉0.05). Conclusion MVD in renal cell carcinoma is higher than that in adjacent tumor tissue and normal renal tissue. MVD is closely correlated with tumor grade, stage and size.
