摘  要:目的探讨急性缺血性脑卒中运动性失语(Broca失语)患者的颅脑磁共振表现。方法选71例急性缺血性脑卒中运动性失语患者,观察其磁共振(MRI)检查结果,记录其梗塞灶部位,并采用波士顿诊断性失语检查严重程度分级标准进行失语症严重程度分级,且应用北京医科大学第一医院神经内科汉语失语检查法(ABC)中的利手评定标准进行利手评定。结果研究发现21例(29.6%)患者的病变部位位于经典Broca区,50例(70.4%)病灶不在经典Broca区。病灶累及经典语言中枢患者中,0~1级失语患者占71.4%,而累及非经典语言中枢患者中,0~1级失语患者占10%,差异有统计学意义(χ2=27.58,P=0.000)。结论急性脑卒中患者的Broca失语症与神经影像学检查所见的病变部位的关系并不完全符合经典的失语症模式,经典Broca区以外的更多部位都可能参与Broca失语症的形成,但经典Broca区重度失语患者较多。Objective To reveal the MRI features of acute stroke patients with Broca aphasia.Methods 71 acute stroke patients with Broca aphasia underwent MRI examinations and MRI lesions were recorded.Then patients were examined by the Boston diagnostic aphasia examination for severity grading.Handedness evaluation was also performed using the Aphasia Battery of Chinese(ABC) developed by the Neurology Department of the First Hospital of Beijng Medical University.Results The study found 21(29.6%) patients with lesions in the classic Broca area,and 50(70.4%) patients with lesions outside the classic Broca centers.About 71.4% of the patients with lesions in the classic Broca area were Boston diagnostic aphasia at grade 0 ~ 1,which was significantly higher than those lesions outside the classic Broca area(71.4% vs 10%,χ2= 27.58,P = 0.000).Conclusions The relationship between Broca aphasia after acute ischemic stroke and lesion locations does not always accord with the classical clinical anatomic correlations as identified by MRI.Lesions outside the classic language centers might also participate in the development of Broca aphasia,but these with lesions at the Broca area have more severe aphasia.
