摘 要:目的研究生脉注射液对大鼠局灶性脑缺血再灌注损伤的影响。方法雄性SD大鼠80只,体重250~300 g,随机分为5组(n=16):I组(假手术对照组)、Ⅱ组(脑缺血再灌注组)、Ⅲ组(生脉预处理组)、Ⅳ组(生脉后处理组)和Ⅴ组(生脉对照组)。Ⅱ组、Ⅲ组、Ⅳ组大鼠采用线栓法进行脑缺血2 h。Ⅲ组大鼠线栓成功前60 min给予腹腔注射生脉15 mL/kg,Ⅳ组大鼠线栓成功后10 min给予腹腔注射生脉15 mL/kg。再灌注24 h时进行神经功能障碍评分,随后取脑组织,测定脑梗死灶体积、细胞凋亡率、Bcl-2及Bax表达,并且计算Bcl-2与Bax的比值(Bcl-2/Bax)。结果Ⅰ组和Ⅴ组相比,神经功能障碍评分、脑梗死灶体积、细胞凋亡率、Bcl-2及Bax表达、Bcl-2与Bax比值差异无统计学意义(P〉0.05)。与Ⅰ组和Ⅴ组比较,Ⅱ组、Ⅲ组、Ⅳ组神经功能缺陷评分、细胞凋亡率、Bcl-2及Bax表达均升高,Bcl-2/Bax降低,脑梗死灶体积增大,差异有统计学意义(P〈0.01)。与Ⅱ组比较,Ⅲ组和Ⅳ组神经功能障碍评分降低,Ⅲ组和Ⅳ组细胞凋亡率和Bax表达降低,脑梗死灶体积缩小,Bcl-2表达和Bcl-2/Bax升高,差异有统计学意义(P〈0.05或P〈0.01)。与Ⅳ组比较,Ⅲ组脑梗死体积缩小,细胞凋亡率和Bax表达降低,Bcl-2表达和Bcl-2/Bax升高,差异有统计学意义(P〈0.05或P〈0.01)。结论生脉注射液预处理和后处理可减轻大鼠局灶性脑缺血再灌注损伤,其机制与上调Bcl-2表达、下调Bax表达从而降低细胞凋亡的发生有关;且生脉注射液预处理较后处理效果更佳。Objective To study the effect of shengmai injection on focal cerebral ischemia-reperfusion injury in rats.Methods Eighty male SD rats,250 ~ 300 g,were randomly divided into five groups,with 16 rats in each group: group I(sham-operation control group),group Ⅱ(cerebral ischemia reperfusion group),group Ⅲ(shengmai pretreatment group),group Ⅳ(shengmai post-treatment group) and group Ⅴ(shengmai control group).Cerebral ischemia was performed for 2 h in group Ⅱ,Ⅲ,Ⅳ by thread embolism method,and group Ⅲ was given intraperitoneal injection with shengmai 15 mL/kg 60 min before thread embolism,group Ⅳ was given intraperitoneal injection with shengmai 15 mL/kg 10 min after thread embolism.Twenty-four hours after reperfusion,nerve dysfunction was scored,then the brain tissue was removed,infarct volume,cell apoptosis,Bcl-2 and Bax expression were measured and the ratio of Bcl-2 and Bax(Bcl-2/Bax) was calculated.Results There was no significant difference between group I and Ⅴ for nerve dysfunction score,infarct volume,cell apoptosis,Bcl-2 and Bax expression,the ratio of Bcl-2 and Bax(P 0.05).Compared with group Ⅰ and group Ⅴ,group Ⅱ,group Ⅲ,and group Ⅳ had increased neuropsychological dysfunction score,apoptosis rate,and expression of Bcl-2 and Bax,decreased Bcl-2/Bax,and enlarged cerebral infarction size,and the difference was statistically significant(P 0.01).Compared with group Ⅱ,group Ⅲ and group Ⅳ had decreased neuropsychological dysfunction score,apoptosis rate and the Bax expression,reduced size of cerebral infarction,and increased Bcl-2 expression and Bcl-2/Bax,and the difference was statistically significant(P 0.05 or P 0.01).Compared with group Ⅳ,group Ⅲ had reduced size of cerebral infarction,decreased apoptosis rate and the Bax expression,and increased Bcl-2 expression and Bcl-2/Bax,and the difference was statistically significant(P 0.05 or P 0.01).Conclusions The focal cerebral ischemia reperfusion injury may be alleviated by shengma
