摘 要:目的利用PCR-DGGE方法分析应用变性梯度凝胶电泳技术,分析比较未经治疗和根管治疗失败的慢性根尖周炎患牙根管内的菌群结构。方法收集临床诊断为慢性根尖周炎的病例19例,包括未治疗患牙9例和根管治疗失败患牙10例。利用变性梯度凝胶电泳技术比较分析不同临床疾病状态患牙根管内细菌菌群结构的不同。结果每个样本中均可检测到细菌菌群的存在。未经治疗和根管治疗失败的慢性根尖周炎DGGE条带数差异有统计学意义(P<0.05)。聚类分析以及PCA分析结果发现治疗后和未经治疗的根尖周病患牙感染根管内的微生物菌落结构相互之间是有着明显区别的。结论未经治疗与根管治疗失败的慢性根尖周炎根管内细菌菌丛存在明显差异;不同种属细菌的联合作用是导致根管治疗失败的主要原因。Objective To investigate the bacterial community structure of post - treatment endodontic disease. Methods Nineteen single root canals with chronic apical periodontitis were examined. Nine of them were primary apical periodontitis and the others were post - treatment endodontic disease. The root canal microbiota of teeth was profiled by the PCR - DGGE method and then compared, taking into consideration the banding patterns. Results Bacteria were present in all examined cases. The number of bands, which is indicative of the number of bacterial species, was compared intra - group. The difference was statistically significant ( P 〈 0.01 ). C]ustering methods and principal component analysis of DGGE banding pattern placed the samples according to the two clinical conditions. These findings confirmed that endodontic infections were polymicrobia] and showed that there was significant difference in the predominant bacterial composition be- tween the two clinical conditions. Conclusion The bacterial community structure in the root canals of primary periodontitis is different from those of post - treatment endodontic disease. Bacterial combination may play a role in treatment failures.
