摘 要:[目的]探讨灯盏花素对老年重症急性胰腺炎(SAP)患者肾脏功能的保护作用及机制。[方法]将59例老年SAP患者分为治疗组(30例)、对照组(29例);对照组采用常规治疗,治疗组在常规治疗的基础上加用灯盏花素;观察2组患者治疗前与治疗后第4、7、10天的血、尿淀粉酶(AMY),肌酐(Cr),尿素氮(BUN),24h尿蛋白的变化以及急性肾功能衰竭(ARF)的发生率、患者病死率。[结果]治疗组患者ARF的发生率、患者病死率均较对照组明显下降(均P<0.05)。治疗组治疗后第4天和第7天血AMY、Cr水平,治疗后第4天血BUN、24h尿蛋白含量水平以及治疗后第7天尿AMY水平均显著低于对照组(均P<0.05)。[结论]灯盏花素对老年SAP患者肾脏功能有保护作用,其机制可能与其扩张微血管、改善微循环、降低血黏度、减轻炎症发生发展及降低肾小球基底膜通透性有关。[Objective] To investigate the renal protection effect of breviscapine on senile severe acute pancreatitis (SAP). [Methods]Fifty-nine senile SAP patients were divided into control group (n=29) and treatment group(n= 30). Control group was treated with conventional treatment and treatment group re- ceived similar therapy and breviscapine. The changes of blood and urine amylase (AMY), creatinine (Cr), blood uria nitrogen (BUN), 24 urine protein and the incidence of acute renal failure(ARF),mortality before and after breviseapine treatment 4 d, 7 d, 10 d were observed. FResults]The incidence of ARF,mortality, the levels of blood and urine AMY, Cr, BUN, 24 urine protein were significantly decreased in breviscapine treatment group. The blood AMY and Cr levels in 4 d and 7 d after breviscapine treatment were both much lower than that of control group respectively(P〈0.05). The urine AMY levels in 7 d after brevis- capine treatment were far lower than that of control group (P〈0.05). The BUN and 24 urine protein levels in 4 d after breviscapine treatment were significantly different from those of control group(P〈0.05). [Conclusion]The breviscapine has protective effects on renal function in older patients with SAP, and the mechanism might probably relate to its expansion of capillaries,improvement of microcirculation, reduction of blood viscosity,mitigation of the occurrence and development of inflammation and reducing the permeability of the glomerular basement membrane.
