摘　　要：目的:通过完善对不合理处方的干预措施,促进临床合理用药,提高医疗质量。方法:抽查2010—2012年第二季度(4—6月)的部分门急诊处方,汇集经过药师干预后的处方,将有关问题进行分类汇总、分析。结果:经过统计,不合理用药处方包括诊断与用药不符、剂量频次用法不正确、重复用药、配伍禁忌或存在潜在的药物相互作用等8个方面的问题。结论:在门急诊处方调剂工作中,进行实时的处方干预是完全必要的。OBJECTIVE：To evolve the intervention measures on the irrational prescriptions in order to promote clinical rational use of drug and improve the quality of medical care. METHODS： The outpatient ＆ emergency prescriptions during the second quarter of 2010-2012 （April-June） were sampled for collection of those prescriptions that have undergone intervention from pharmacists, with the relevant problems encountered summarized and analyzed. RESULTS： The statistical analysis revealed that the irrationality lie in 8 aspects including discrepancies between diagnosis and medication, incorrect in frequency, dosage and administration, repeated medication, incompatibility or potential drug interactions. CONCLUSION： It is absolutely necessary to perform real-time intervention on the dispensing of outpatient ＆ emergency prescriptions.
