套筒冠及附着体固位赝复体应用于单侧上颌骨缺损修复的临床效果评价

摘　　要：目的研究套筒冠及附着体固位的阻塞器式赝复体修复单侧上颌骨缺损的临床效果。方法收集自2006年6月—2011年6月17例因肿瘤切除单侧上颌骨的病例,制作利用套筒冠及附着体固位的阻塞器式赝复体,通过吸光度法比较修复前和修复后1个月咀嚼效率变化,研究套筒冠及附着体固位阻塞器式赝复体的临床应用效果。结果临床观察得知,17例患者对套筒冠及附着体固位阻塞器式赝复体均表示满意,语音功能恢复良好,修复前吸光度值为0.385±0.162,戴用1个月后吸光度值为0.544±0.136,差异有显著统计学意义(P<0.05)。结论套筒冠及附着体固位阻塞器式赝复体能较好恢复单侧上颌骨缺损患者的面容和语音功能,戴用后咀嚼效率有明显改善,临床效果满意。Objective To evaluate clinical results of obturator prosthesis by applicating telescopic crowns and attachments in the re- pair of unilateral maxillary defects. Methods Obturator prosthesis was fixed with telescopic crowns and attachments in 17 patients whose unilateral maxillas was resected because of tumor from June 2006 to June 2011. Masticatory efficiency before repairation and one month later were determined and compared by light absorption method. Results For the 17 patients, their self evaluation of clinical effect was excellent,and their speaking functions recovered well. Light absorption value was 0. 385 ±0. 162 before repair while it was 0. 544 ±0. 136 one month later. There was significant difference （ P 〈 0. 05 ）. Conclusions Obturator prosthesis fixed by telescopic crowns and attachments can effectively help recovery of countenance and speaking functions of patients with unilateral maxillary defects. Masticatory efficiency is improved obviously after use and the clinical effects are satisfactory.
